摘 要:目的:探讨黏液样脂肪肉瘤(MLS)的影像学特征,并通过文献复习提高对该病的认识。方法:回顾性分析7例经病理证实的MLS的影像学资料,结合文献对该病的流行病学、病理生理机制、影像学特征、诊断及鉴别诊断、治疗及预后进行系统阐述。结果:4例行超声检查,表现为低回声和无回声肿块,无回声内透声差及多发分隔,实性区内多有不同程度的强回声;3例行CT检查,多表现为囊状低密度影及稍低软组织密度影,内部可见多发分隔;4例行MRI检查,T1WI以等低信号、T2WI高信号为主,脂肪抑制T2WI序列肿块呈低信号,内可见T2WI低信号分隔,增强扫描可见实性成分和粗大的间隔明显强化。结论:超声及CT对MLS的诊断具有一定价值,超声用于该病与囊性病变鉴别,CT图像内部分隔及脂肪成分可提示该病;MRI对该病诊断价值较大,同时可用于术后随访。Objective:To explore the imaging features of myxoid liposarcoma in order to improve the understanding of this disease.Methods:The imaging data of 7 patients with myxoid liposarcoma confirmed by pathology were analyzed retrospectively.The disease epidemiology,pathophysiological mechanism,diagnosis,differential diagnosis,treatment and prognosis of the disease were systematically identified through literature review.Results:The sonographic features of myxoid liposarcoma were hypoechoic mass and anechoic mass with poor acoustic transmission,multiple septations and different degree of hyperechogenicity in the solid area;CT features included cystic hypodensity and slightly low soft tissue density with multiple internal septations.T1WI showed mainly equal low signal and T2WI showed high signal.The signal intensity of the mass on T2WI fat suppression sequence did not decrease,and short T2 internal separations might be visible.On contrast-enhanced scan,the solid components and thick septations were obviously enhanced.Conclusions:Ultrasound and CT have a certain value in the diagnosis of this disease.Ultrasound is used to differentiate it from cystic lesions.The presence of internal separations and fat components in CT images can suggest this disease.MRI is of great value in the imaging diagnosis of the disease and can be used for the postoperative follow-up.
