探析风药对血管性痴呆的精准治疗

摘　　要：在导致痴呆的病因中,血管性痴呆(vascular dementia,VaD)仅次于阿尔茨海默病(Alzheimer’s disease,AD)居第二位,且发病率有逐年上升的趋势,因其具有可预防性,探析血管性痴呆的治疗变得尤为重要,目前西医暂没有针对性的治疗药物,而祖国医学在血管性痴呆的治疗中具有明显的优势。在临床实践中精确辩证,合理配伍风药可获得良好的疗效。风药有升、散、动、窜的特性,具有升阳、胜湿、疏肝理气、活血化瘀、开通玄府、引经报使的功效,与血管性痴呆的病因病机相对应,探析风药的不同功效在血管性痴呆肾精亏虚型、痰浊阻窍型、肝阳上亢型、瘀血阻络型中的精准治疗,总结风药在血管性痴呆中的应用规律,并结合大量实验研究、临床案例,列举川芎、葛根、天麻3种风药及其相关方药、制剂的现代药理及临床应用,为临床使用风药治疗血管性痴呆提供理论依据,进一步为血管性痴呆的中医疗法提供有意义的借鉴。Among the causes of dementia,vascular dementia(VaD) is second only to Alzheimer’s disease(AD),and its incidence rate has a rising trend year by year.Because of its preventive nature,it is particularly important to explore the treatment of VaD.Currently,western medicine has no specific therapeutic drugs,while traditional Chinese medicine has obvious advantages in the treatment of VaD.In clinical practice,good curative effect can be obtained by accurate dialectics and reasonable compatibility of Wind-dispelling drugs.wind-dispelling drugs have the characteristics of ascending,dispersing,moving and dredging,and has the effect of ascending Yang,eliminating dampness,soothing liver and regulating Qi,promoting blood circulation and removing blood stasis,opening Xuanfu and inducing menstruation.Corresponding to the etiology and pathogenesis of VaD,this paper explored the precise treatment of different effects of wind-dispelling drugs in VaD of kidney essence deficiency type,phlegm obstruction type,liver Yang hyperactivity type and blood stasis obstruction type,summarized the application rules of wind-dispelling drugsand combined with a large number of experimental studies and clinical cases.The modern pharmacology and clinical application of three wind-dispelling drugs included Chuanxiong(Rhizoma Chuanxiong),Gegen(Radix Puerariae) and Tianma(Rhizoma Gastrodiae) and their related prescriptions and preparations were listed so as to provide theoretical basis for the clinical use of wind-dispelling drugs in the treatment of VaD and meaningful reference for the traditional Chinese medicine treatment of VaD.
