腮腺浅叶部分切除术治疗腮腺多形性腺瘤的临床研究

摘　　要：目的探讨腮腺浅叶部分切除术(PSP)与腮腺浅叶切除术(SP)对腮腺多形性腺瘤患者腮腺分泌功能、外貌负性情绪及身体意象状态的影响。方法选取我院收治的腮腺多形性腺瘤患者92例,根据不同的手术方法分为SP组(n=45,给予SP治疗)和PSP组(n=47,给予PSP治疗)。比较2组手术相关指标、腮腺分泌功能、外貌负性情绪、身体意象状态、并发症及复发情况。结果PSP组手术时间、切口长度短于SP组(P<0.05),术中出血量、解剖面神经分支数少于SP组(P<0.05)。2组术后3个月、6个月、12个月腮腺分泌功能的分泌差均较术前增加(P<0.05);2组术后3个月腮腺分泌功能的分泌差比较,差异无统计学意义(P>0.05);PSP组术后6个月、12个月腮腺分泌功能的分泌差均低于SP组(P<0.05)。2组术后6个月体像担忧问卷(BICI)、身体意象量表(BIS)评分均较术前增加,但PSP组均低于SP组(P<0.05);2组术后12个月BICI、BIS评分比较,差异无统计学意义(P>0.05)。PSP组并发症发生率低于SP组(P<0.05);2组复发率比较,差异无统计学意义(P>0.05)。结论与SP相比,PSP治疗腮腺多形性腺瘤患者,可优化手术相关指标,并改善其身体意象状态和腮腺分泌功能,减轻外貌负性情绪,降低术后并发症发生风险,且不增加复发率。Objective To investigate the effects of partial superficial parotidectomy(PSP)and superficial parotidectomy(SP)on parotid gland secretion function,appearance negative emotion and body image state of patients with parotid pleomorphic adenoma.Methods A total of 92 patients with parotid pleomorphic adenoma admitted to our hospital were selected and divided into the SP group(n=45,treated with SP)and the PSP group(n=47,treated with PSP)according to different surgical methods.The operation-related indexes,parotid gland secretion function,appearance negative emotion,body image state,complications and recurrence rates between the two groups were compared.Results The operation time and incision length of the PSP group were shorter than those of the SP group(P<0.05),the volume of intraoperative blood loss and the number of anatomical facial nerve branches were less than those of the SP group(P<0.05).The secretory difference of the parotid gland secretion function in the two groups 3 months,6 months and 12 months after operation were higher than those before the operation(P<0.05),and there was no statistically significant difference in secretory difference of parotid gland secretion function between the two groups 3 months after operation(P>0.05).The secretory differences of parotid gland secretion function 6 months and 12 months after operation in the PSP group were lower than those in the SP group(P<0.05).The scores of body image concern inventory(BICI)and body image scale(BIS)of the two groups 6 months after operation were higher than those before the operation,which of the PSP group were lower than those of the SP group(P<0.05).There was no statistically significant difference in the scores of BICI and BIS between the two groups 12 months after operation(P>0.05).The incidence of complications in the PSP group was lower than that in the SP group(P<0.05).There was no statistically significant difference in the recurrence rate between the two groups(P>0.05).ConclusionCompared with the SP,PSP in the treatment of the patient
