Effects of Modified Yinchenhao Decoction(茵陈蒿汤)on Intestinal Microecology in Patients with NAFLD of Damp-Heat Syndrome

摘  要:目的探究茵陈蒿汤加减对非酒精性脂肪肝(NAFLD)湿热证患者肠道微生态的影响。方法将96例NAFLD湿热证患者随机分为对照组和观察组各48例,对照组给予常规西医治疗,观察组患者在常规西医治疗基础上应用茵陈蒿汤加减治疗,比较两组患者疗效、肝功能与血脂代谢、肠道微生态、血清白细胞介素-10(IL-10)、干扰素-γ(IFN-γ)水平。结果观察组和对照组患者治疗总有效率分别为95.83(46/48)%和83.33%(42/48),差异有统计学意义(P<0.05);治疗后,两组患者肝功能[丙氨酸转氨酶(ALT)、天门冬氨酸氨基转移酶(AST)和谷氨酰转肽酶(GGT)]、血脂指标[总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)]及IFN-γ水平较治疗前降低,高密度脂蛋白胆固醇(HDL-C)、IL-10水平较治疗前升高,条件致病菌(肠杆菌、肠球菌)数量较治疗前减少,益生菌(双歧杆菌、乳酸杆菌)数量较治疗前增加,且观察组患者肝功能指标及TC、TG、LDL-C、IFN-γ水平低于对照组,HDL-C、IL-10水平高于对照组,肠杆菌与肠球菌数量少于对照组,双歧杆菌与乳酸菌数量多于对照组,差异有统计学意义(P<0.05)。结论茵陈蒿汤加减对非酒精性脂肪肝湿热证疗效显著,可保护患者肝功能、改善血脂代谢及肠道微生态环境,调节血清IL-10、IFN-γ水平。Objective To explore the effects of Modified Yinchenhao Decoction(茵陈蒿汤)on intestinal microecology in patients with nonalcoholic fatty liver disease(NAFLD)of damp-heat syndrome.Methods Ninety-six patients with NAFLD of damp-heat syndrome were randomly divided into control group and observation group,with 48 cases in each group.The control group was given conventional Western medicine treatment,and the observation group was treated with Modified Yinchenhao Decoction on the basis of conventional Western medicine treatment.The efficacy,liver function and blood lipid metabolism,intestinal microecology and levels of serum interleukin-10(IL-10)and Interferon-γ(IFN-γ)were compared between the two groups.Results The total effective rates in the observation group and control group were 95.83%(46/48)and 83.33%(42/48)respectively(P<0.05).After treatment,the levels of liver function indexes[alanine aminotransferase(ALT),aspartate aminotransferase(AST),glutamyl transpeptidase(GGT)],blood lipid indexes[total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C)]and IFN-γin the two groups were lower than those before treatment while the levels of high-density lipoprotein cholesterol(HDL-C)and IL-10 were higher than those before treatment,and the quantities of conditioned pathogens(enterobacteria,enterococci)were decreased compared with those before treatment while the quantities of probiotics(bifidobacteria,lactobacilli)were increased compared with those before treatment,and the levels of liver function indexes and TC,TG,LDL-C and IFN-γin the observation group were lower than those in the control group while the levels of HDL-C and IL-10 were higher than those in control group,and the quantities of enterobacteria and enterococci were less than those in control group while the quantities of bifidobacteria and lactobacilli were more than those in control group(P<0.05).Conclusion Modified Yinchenhao Decoction has a significant efficacy on nonalcoholic fatty liver disease of damp-heat syndrome,and i
