白内障行超声乳化术中后囊破裂原因分析

摘　　要：目的分析白内障行晶状体超声乳化术术中后囊破裂的原因。方法回顾性分析马山县人民医院2016年1月至2019年12月共1 650例(1 650眼)白内障患者行超声乳化人工晶状体植入术术中后囊破裂57例(57眼)的临床资料。分析术中后囊破裂的原因。结果后囊破裂的原因包括撕囊不当5例(8.8%),水分离不当2例(3.5%),劈核不当10例(17.5%),旋转核不当13例(22.8%),吸附抓核不当10例(17.5%),超声乳化核不当11例(19.3%),吸除皮质不当5例(8.8%),植入人工晶状体不当1例(1.8%)。结论超声乳化术中后囊破裂多发生于处理核时。Objective To analyze the causes of posterior capsule rupture during phacoemulsification for cataract.Methods The clinical data of 1650 eyes of 1650 cases of cataract treated with phacoemulsification from Jan.2016 to Dec.2019 in Mashan County People’s Hospital were analyzed retrospectively.Among these patients,57 eyes of 57 cases occurred posterior capsule rupture during the surgery.The causes of intraoperative posterior capsule rupture were analyzed.Results The causes of posterior capsule rupture included 5 cases(8.8%)of inappropriate capsulorhexis,2 cases(3.5%)of improper hydrodissection,10 cases(17.5%)of inappropriate chop,13 cases(22.8%)of spun nucleus inappropriately,10 cases(17.5%)of improper nucleus suction and grip,11 cases(19.3%)of improper nucleus phacoemulsification,5 cases(8.8%)of improper cortex suction,and 1 case(1.8%)of inappropriate intraocular lens implantation.Conclusion In phacoemulsification,posterior capsule rupture mostly occurs when the lens nucleus is treated.
