外伤性青光眼Ex-Press引流器植入术的疗效观察

摘　　要：目的观察Ex-Press引流器植入术治疗外伤性青光眼的效果。方法回顾性研究。收集2019年5月至2020年7月收治于厦门大学附属厦门眼科中心的外伤后继发性青光眼者30例(30眼)施行Ex-Press引流器植入术,随访3~12个月,观察术后眼压及视力变化。结果术前眼压为(40.19±7.76)mmHg(1mmHg=0.133 kPa),术后1 d、1周、1个月及3个月眼压分别为(13.40±5.27)mmHg、(13.26±5.01)mmHg、(13.88±5.28)mmHg及(13.55±2.91)mmHg,与术前相比均有明显下降(t=-21.25,-20.48,-22.04,-20.88;P均<0.05)。术后各个随访时间点之间眼压差异均无统计学意义(P>0.05)。10眼(33.33%)术后视力不同程度提高,其中1眼BCVA由0.3提高至0.7。结论 Ex-Press引流器治疗外伤性青光眼效果显著,操作简单,术后眼压稳定。Objective To observe the efficacy of Ex-Press glaucoma drainage device for the treatment of traumatic glaucoma.Methods This was a retrospective study.The data of 30 eyes of 30 patients with traumatic glaucoma in Xiamen Eye Center Affiliated of Xiamen University from May 2019 to Jul.2020 were collected.The changes of intraocular pressure(IOP)and best corrected visual acuity(BCVA)were observed before and after operation.The follow-up period was 3-12 months.Results The preoperative intraocular pressure(IOP)was(40.19±7.76)mmHg(1 mmHg=0.133 kPa).And the postoperative IOP at 1 day,1 week,1 month and 3 months were(13.40±5.27)mmHg,(13.26±5.01)mmHg,(13.88±5.28)mmHg and(13.55±2.91)mmHg respectively.Postoperative IOP at every time point were lower than that preoperation(t=-21.25,-20.48,-22.04,-20.88;all P<0.05).There was no significant difference in IOP after surgery among each follow-up time point(P>0.05).The postoperative visual acuity was improved in varying degrees in 10 eyes(33.33%),of which the BCVA was improved from 0.3 to 0.7 in 1 eye.Conclusion Ex-Press glaucoma drainage device is effective for the treatment of traumatic glaucoma with simple operation and stable IOP.
