多学科诊疗下药代动力学指导重型血友病A患者截骨矫形术个体化用药1例

摘　　要：药代动力学(PK)指导围术期凝血Ⅷ因子使用是近年出现的血友病新兴管理思路。南方医院多学科诊疗团队紧密协作,运用PK指导1例“右膝外翻畸形”的重型血友病A患者围术期凝血Ⅷ因子使用,根据其长半衰期(20.17 h)延长给药间隔,并制定个体化“截骨矫形术”方案。患者术程顺利,与基于体质量的截骨手术围术期凝血Ⅷ因子用量相比共节省34.4%用药量。本例旨在为血友病手术患者提供PK指导下围术期管理新思路。Pharmacokinetics(PK)to guide the use of perioperative coagulation factorⅧis an emerging management approach for hemophilia in recent years.The Multi-disciplinary Treatment(MDT)team of Nanfang Hospital worked closely and used PK to guide the use of FⅧin a patient with severe hemophilia A with"right knee valgus deformity"during the perioperative period.The dosing interval was extended according to the patient's long half-life(20.17 h),and develop an individualized"osteotomy orthopedic"plan.The patient's procedure was smooth.Compared with the FⅧdosage during the perioperative period of weightbased osteotomy,a total of 34.4%FⅧwas saved.This case aims to provide new ideas for perioperative management under the guidance of PK for surgical patients with hemophilia.
