新辅助化疗联合保乳手术治疗中晚期乳腺癌的临床效果分析

摘　　要：目的探讨中晚期乳腺癌应用新辅助化疗联合保乳手术治疗的临床效果。方法78例中晚期乳腺癌患者,以抽签形式分成对照组与实验组,每组39例。对照组患者采用改良根治术,实验组患者采用新辅助化疗联合保乳手术治疗。比较两组患者治疗效果、乳房外形美观优良率、手术指标、生活质量。结果实验组治疗总有效率87.18%高于对照组的64.10%,差异具有统计学意义(P<0.05)。实验组患者乳房外形美观优良率94.87%高于对照组的74.36%,差异具有统计学意义(P<0.05)。实验组患者手术时间(44.62±8.67)min、住院时间(9.62±1.33)d均短于对照组的(77.36±10.75)min、(15.47±2.52)d,术中出血量(125.84±13.42)ml少于对照组的(226.52±20.71)ml,差异均具有统计学意义(P<0.05)。实验组患者生理功能、环境、心理功能、社会关系评分分别为(93.52±2.74)、(93.65±2.32)、(92.85±2.45)、(93.18±2.73)分,均高于对照组的(84.68±2.41)、(83.87±2.62)、(83.66±2.16)、(83.49±2.68)分,差异均具有统计学意义(P<0.05)。结论对中晚期乳腺癌患者采用新辅助化疗联合保乳手术治疗效果较理想,可以缩短患者住院时间,提高患者生活质量,促进患者快速恢复。Objective To discuss the clinical effect of neoadjuvant chemotherapy combined with breastconserving surgery for advanced breast cancer.Methods A total of 78 patients with advanced breast cancer were divided into control group and experimental group according to lottery method,with 39 cases in each group.The control group was treated with modified radical mastectomy,and the experimental group was treated with neoadjuvant chemotherapy combined with breast-conserving surgery.The therapeutic effect,excellent and good rate of breast appearance,surgical indicators and quality of life were compared between the two groups.Results The total effective rate 87.18%of the experimental group was higher than 64.10%of the control group,and the difference was statistically significant(P<0.05).The excellent and good rate of breast appearance 94.87%of the experimental group was higher than 74.36%of the control group,and the difference was statistically significant(P<0.05).The operation time(44.62±8.67)min and hospitalization time(9.62±1.33)d of the experimental group were shorter than(77.36±10.75)min and(15.47±2.52)d of the control group,and intraoperative blood loss(125.84±13.42)ml was less than(226.52±20.71)ml of the control group.All the difference was statistically significant(P<0.05).The physiological function,environment,psychological function,social relationship score of the experimental group were(93.52±2.74),(93.65±2.32),(92.85±2.45)and(93.18±2.73)points,which were higher than(84.68±2.41),(83.87±2.62),(83.66±2.16)and(83.49±2.68)points of the control group,and the difference was statistically significant(P<0.05).Conclusion Combination of neoadjuvant chemotherapy and breast-conserving surgery shows ideal effect on patients with advanced breast cancer,and it can shorten hospitalization time,improve the quality of life and promote the rapid recovery of patients.
