摘  要:目的应用功能性近红外光谱成像技术(fNIRS)研究不同级别病理性近视(PM)患者在高、低两种空间频率棋盘格视觉刺激任务下的大脑视皮层活动。方法受试者分为3组:PM-A组(PM患者中合并2级、3级近视性黄斑病变)17眼、PM-B组(PM患者中合并4级近视性黄斑病变)11眼及正常对照组(NC组)16眼。受试者于安静、昏暗的房间中依次观看低空间频率、高空间频率的黑白棋盘格翻转刺激任务,视觉刺激同时用ETG-4000系统检测其氧合血红蛋白(Oxy-Hb)浓度的变化。使用MATLAB及SPSS 17.0软件包对数据进行处理分析。结果三组受试者的性别构成、年龄、眼压等基线资料差异均无统计学意义(均为P>0.05)。在低空间频率刺激任务下,NC组与PM-A组、PM\|B组的Δβ值(β刺激期-β基线期)差异均有统计学意义(t=2.462、2.091,P=0.020、0.047),PM-A组与PM-B组的Δβ值差异无统计学意义(t=-0.633,P=0.532)。在高空间频率刺激任务下,NC组与PM-A组、PM\|B组的Δβ值差异均有统计学意义(t=2.651、3.051,P=0.013、0.005),PM-A组与PM-B组的Δβ值差异无统计学意义(t=0.524,P=0.605)。PM-A组及PM-B组患者Oxy-Hb的激活量在低空间频率及高空间频率任务中均显著低于NC组。NC组、PM-A组及PM-B组各组内受试者在低空间频率与高空间频率刺激任务下,大脑Oxy-Hb的激活量差异均无统计学意义(t=0.692、-0.285、1.226,P=0.499、0.779、0.248)。结论作为一种晚发性眼病,PM长期异常的视觉经验及眼底病变可引起脑皮层功能活动的减弱。PM患者视皮层功能活动在低空间频率及高空间频率棋盘格刺激时均存在降低。fNIRS可为PM等视路相关疾病患者的脑功能研究提供依据。Objective To detect the visual cortical activity of patients with different levels of pathological myopia(PM)with checkerboard visual stimulation of low or high spatial frequency by the functional near-infrared spectroscopy(fNIRS).Methods The subjects were divided into three groups:PM-A group(macular lesions of grade 2 and grade 3)which included 17 eyes,PM-B group(macular lesions of grade 4)which contained 11 eyes and normal control group(NC group)which included 16 eyes.In a quiet and dim room,all subjects received checkerboard reversal stimulations with a sequence of low and high spatial frequency.The cortical oxyhemoglobin levels were detected by ETG-4000 synchronously with the visual stimulation.MATLAB and SPSS 17.0 software were used to analyze the data.Results There were no statistically significant differences in the gender composition ratio,age,intraocular pressure and other baseline information among the three groups(all P>0.05).Under the low spatial frequency stimulation task,there were statistically significant differences inΔβvalues(β_(stimulation period)-β_(baseline period))between NC group and PM-A group,as well as between NC group and PM-B group(t=2.462,2.091,P=0.020,0.047).There was no statistical difference inΔβvalue between the PM-A group and PM-B group(t=-0.633,P=0.532).Under the high spatial frequency stimulation task,there were statistically significant differences inΔβvalue between NC group and PM-A group,as well as between NC group and PM-B group(t=2.651,3.051,P=0.013,0.005).There was no statistical difference inΔβvalue between PM-A group and PM-B group(t=0.524,P=0.605).The levels of oxyhemoglobin in the PM-A and PM-B groups were significantly lower than those of NC group,whether in low spatial frequency or high spatial frequency visual stimulation.There were no statistically significant differences in the Oxyhemoglobin levels in the visual cortex when subjects received stimulation of low or high spatial frequency in PM-A group,PM-B group,and NC group(t=0.692,-0.285,1.226,P=0.499,0
