艾司奥美拉唑肠溶片不同剂量及口服频次治疗幽门螺杆菌阴性胃术后吻合口溃疡临床疗效观察

摘　　要：[目的]观察不同剂量及不同口服频次艾司奥美拉唑肠溶片治疗幽门螺杆菌(Hp)阴性胃术后吻合口溃疡临床效果和安全性。[方法]选取在我院诊断Hp阴性胃术后吻合口溃疡84例,按随机数字表法分为甲组和乙组,每组42例。2组均口服艾司奥林拉唑肠溶片(阿斯利康制药有限公司生产,国药准字H20046379),疗程均4周。甲组40 mg/次,1次/d(晨起);乙组20 mg/次,2次/d(晨起+睡前)。对2组患者的临床症状缓解率和胃镜下溃疡愈合率进行分析对比。[结果]乙组治疗近期腹痛临床症状缓解率优于甲组(P<0.01),2组远期效果相当(P>0.05)。胃镜观察溃疡愈合情况,2组有效率相同,差异无统计学意义(P>0.05)。[结论]艾司奥美拉唑肠溶片治疗Hp阴性胃术后吻合口溃疡具有明显的疗效,且艾司奥美拉唑肠溶片20 mg,2次/d近期对临床症状缓解有效率高于艾司奥美拉唑肠溶片40 mg,1次/d。[Objective]To observe the clinical effect and safety of esomeprazole enteric-coated tablets in different doses and oral frequency in the treatment of anastomotic ulcers after Helicobacter pylori(Hp)-negative gastric surgery.[Methods]A total of 84 cases of anastomotic ulcers after Hp-negative gastric surgery were diagnosed in our hospital from February 2016 to August 2020.They were divided into group A and group B according to a random number table,with 42 cases in each group.Esomeprazole enteric-coated tablets(manufactured by AstraZeneca Pharmaceutical Co.,Ltd.,National Medicine Zhunzi H20046379)were taken orally in both groups,and esomeprazole enteric-coated tablets 40 mg/time,1 time/d(morning Beginning),the course of treatment was 4 weeks.Group B takes orally esomeprazole enteric-coated tablets 20 mg/time,2 times/d(morning+before going to bed),treatment course is 4 weeks.The clinical symptom relief rate and gastroscopic ulcer healing rate of the two groups were analyzed and compared.[Results]Esomeprazole enteric-coated tablets 20 mg,2 times/d,in the treatment of short-term abdominal pain,the clinical symptom relief rate was better than esomeprazole enteric-coated tablets 40 mg,1 time/d,and the long-term effect was equivalent.Observing the healing of ulcers under gastroscopy,the two groups had the same effective rate,and the difference was not statistically significant.[Conclusion]Esomeprazole enteric-coated tablets have obvious curative effect in the treatment of anastomotic ulcers after Hp-negative gastric surgery,and esomeprazole enteric-coated tablets 20 mg,2 times/d,have a higher effective rate of clinical symptom relief in the near future esomeprazole enteric-coated tablets 40 mg,once daily.It provides a theoretical basis for the further.
