摘　　要：[目的]探讨血清CA19-9动态变化及血清酶学比值对胆胰疾病的诊断价值。[方法]回顾性分析126例胆胰疾病的住院病例资料,统计分析血清CA19-9动态变化及其与血清酶学比值等指标。[结果](1)CA19-9在胆胰疾病有不同程度的升高,良性疾病多≤200 U/ml(达80%以上);而恶性疾病多较高,>200 U/ml达59.1%。(2)CA19-9与血清酶学之比在不同的胆胰疾病中有差异(P=0.00),在恶性胆胰疾病中更高(P<0.05),比值≥1约占65%。大部分CA19-9比血清酶学上升更明显。(3)动态观察CA19-9水平示良性胆胰疾病短期内CA19-9水平呈下降趋势;而恶性胆胰疾病CA19-9水平呈上升趋势或改变不明显(χ2=50.94,P=0.000)。[结论]CA19-9在良、恶性胆胰疾病中有不同程度的升高;CA19-9与血清酶学比值可作为在诊断良、恶性胆胰疾病的另一项补充;CA19-9的动态变化对诊断良、恶性胆胰疾病有重要价值。[Objective]To investigate the diagnostic value of serum CA19-9 dynamics and serum enzymology ratio for biliary and pancreatic diseases.[Methods]The totals of 126 cases of hospitalized cases of biliary and pancreatic diseases were retrospectively analyzed,the serum CA19-9 and its dynamic changes and serum enzymes ratio and other indicators were also evaluated.[Results](1)CA19-9 had different degrees of increase in biliary and pancreatic diseases.More than 80%of benign diseases were less than 200 U/ml;and up to 59.1%of malignant diseases were more than 200 U/ml.(2)The ratio of CA19-9 to serum enzymology was different in different biliary and pancreatic diseases(P=0.00),higher in biliary and pancreatic malignant diseases(P<0.05),and the ratio was greater than 1 in about 65%of cases.Part of CA19-9 was more pronounced than serum enzymology.(3)Dynamic observation of CA19-9 levels,biliary and pancreatic inflammatory diseases showed a downward trend in the short term;while biliary and pancreatic malignant diseases showed an upward trend or no change(χ2=50.94,P=0.000).[Conclusion]CA19-9 has different degrees of elevation in benign and malignant biliary and pancreatic diseases;CA19-9 and serum enzymology ratio can be used as another supplement in the diagnosis of benign and malignant biliary and pancreatic diseases;dynamic changes of CA19-9 is of great value on diagnosis of benign and malignant biliary and pancreatic diseases.
