梭形皮肤切除术与Wheeler术治疗下睑内翻患者效果比较

摘　　要：目的比较外眦部梭形皮肤切除术(Fox术)与眼轮匝肌缩短术(Wheeler术)治疗下睑内翻患者效果。方法选取我院2014年7月~2017年7月下睑内翻患者256例,依据手术方式不同分为Fox术组(128例)、Wheeler术组(128例),Fox术组行Fox术,Wheeler术组行Wheeler术,统计对比两组疗效及术后18个月、24个月复发率、术后24个月美观满意度。结果Fox术组总有效率92.19%(118/128)与Wheeler术组92.97%(119/128)对比,差异不明显(x^(2)=0.057,P>0.05);术后18个月、24个月Fox术组复发率8.73%(11/126)、11.20%(14/125)低于Wheeler术组22.83%(29/127)、30.95%.(39/126,x_(1)^(2)=9.453,x_(2)^(2)=14.698,P<0.05);术后24个月两组美观满意度对比,Fox术组91.20%(114/125)高于Wheeler术组76.19%(96/126,x_(1)^(2)=10.344,P<0.05)。结论Fox术与Wheeler术治疗下睑内翻疗效.Objetive To compare the effect of fusiform cutaneous resection(Fox surgery)and orbicularis oculi muscle shortening(Wheeler surgery)in the treatment of patients with lower eyelid inversion.Methods A total of 256 patients with lower eyelid varus were selected in our hospital from July 2014 to July 2017,they were divided into Fox surgery group(128 cases)and Wheeler surgery group(128 cases)according to different surgical methods.Fox surgery group performed Fox surgery and Wheeler surgery group performed Wheeler surgery,the curative effect,recurrence rates at 18 and 24 months after surgery,and aesthetic satisfaction at 24 months after surgery were statistically compared between the two groups.Results The total effective rate of the Fox surgery group was 92.19%(118/128)compared with 92.97%(119/128)in the Wheeler surgery group,the difference was not significant(x^(2)=0.057,P>0.05).At 18 and 24 months after surgery,the recurrence rate in the Fox operation group was 8.73%(11/126)and 11.20%(14/125),which were lower than the 22.83%(29/127,x_(1)^(2)=9.453)and 30.95%(39/126,x_(2)^(2)=14.698)in the Wheeler surgery group(P<0.05);The aesthetic satisfaction at 24 months after surgery between the two groups were compared,91.20%(114/125)in the Fox surgery group was higher than 76.19%(96/126)in the Wheeler surgery group(x_(1)^(2)=10.344,P<0.05).Conclusion Fox and Wheeler surgery are effective in the treatment of lower eyelid inversion,but Fox surgery can significantly reduce the long-term recurrence rate and keep the eyes beautiful for a long time.
