高职护理实习生临终护理真实体验的质性研究

摘　　要：目的了解高职护理实习生在实施临终护理期间的真实体验,为高职护理死亡教育改革提供参考。方法2019年11月至2020年1月选取顺德职业技术学院2017级高职护理实习生为研究对象,运用质性研究中的现象学方法对20名高职护理实习生进行半结构式深入访谈,运用Colaizzi分析法对访谈结果进行整理分析。结果提炼出4个主题:临终护理的情绪变化(面对患者死亡、面对家属、面对死亡习俗仪式的情绪变化)、自我调适与反思(负性情绪自我调适、接触死亡后反思)、对临终护理的认知和意愿(对临终护理工作的认知、择业意愿)、对死亡教育的需求和建议(对学校教育、临床实习的需求和建议)。高职护理实习生在临终护理过程中会出现复杂的情绪变化,大多选择积极的自我调适方式;临终护理知识技能不足;择业意愿较低;渴望获得更多有关教育支持。结论建议加强高职护生在校与实习两阶段的死亡和临终护理教育,以适应未来护理人才发展要求。Objective To explore the real experience of hospice care in higher vocational nursing students in the period of internship and provide advice for the reform of death education in higher vocational schools.Methods A qualitative research was conducted by means of semi-structured in depth interview with 20 higher vocational nursing students from Shunde Polytechnic during Nov.2019 to Jan.2020 with method of phenomenology being used.Colaizzi approach was performed to analyze the results of interview.Results Four themes were extracted in total,including emotional changes during hospice care(facing with the death,relations and custom),self-adjustment and reflection(negative emotion and reflection on death),cognition and willingness of hospice care(career preference),needs and recommendations for death education.Higher vocational nursing students experience complicated emotional changes in hospice care.However,most of them choose positive self-adjustment method.With insufficient skills and lower willingness of hospice care,they desire for more education support.Conclusions The education of death and hospice care should be strengthened both in schooling and internship,in order to adapt to the development of nursing services.
