保留上颌窦假性囊肿上颌窦底侧壁开窗提升同期种植体植入术的临床观察

摘　　要：目的探讨保留上颌窦假性囊肿同期行上颌窦侧壁开窗提升种植体植入术的效果,为临床实践提供参考。方法按照纳入病例是否伴有囊肿分为囊肿组23例植入种植体31枚,与非囊肿组23例植入种植体32枚,囊肿组行保留假性囊肿的上颌窦底侧壁开窗提升同期种植体植入术,非囊肿行不伴有上颌窦假性囊肿的上颌窦侧壁开窗提升种植术,对比观察囊肿组与非囊肿组术前剩余骨高度(RBH),术后即刻、6个月、1年种植体根方骨高度,种植体存留率,术前术后囊肿变化等指标,评估上颌窦侧壁开窗提升术对囊肿的影响,及囊肿的存在是否会增加上颌窦侧壁开窗提升术并发症的发生率,同时观察囊肿对种植体根方成骨是否有影响。结果2组患者RBH术后即刻、6个月、1年时种植体根方平均骨高度差异无统计学意义,不同时期囊肿大小差异具有统计学意义(P<0.05),术后即刻较术后6个月差异无统计学意义(P>0.05),两者之间均值差为141.48 mm^(2)。术后1年囊肿大小较术前差异有统计学意义(P<0.05),两者之间均值差为-130.24 mm^(2)。术后随访期内囊肿组、非囊肿组均无上颌窦黏膜穿孔的病例,未出现鼻腔渗血、感染及上颌窦炎,植骨区创口愈合良好,种植体累计存留率100%。结论上颌窦侧壁开窗提升术不影响囊肿自然发展过程。上颌窦假性囊肿并不影响种植体根方成骨。保留上颌窦假性行上颌窦侧壁开窗同期种植体植入的手术方式切实可行。Objective To observe the effect of lateral wall fenestration of maxillary sinus on implant implantation for clinical practice.MethodsAccording to whether there were cysts in the cases included,the cyst group was divided into cyst group and non-cyst group.The patients in cyst group were performed implant implantation of maxillary sinus floor lateral wall fenestration without cyst of maxillary sinus pseudocyst.At the same time,the effect of cyst on root osteogenesis was observed.ResultsThere were 23 patients in the cyst group,31 implants were implanted;and 32 implants were implanted in 23 patients in the non-cystic group.There was no differences in age and sex between the cyst group and the non-cystic group.There were no significant differences in RBH,bone height after operation and the mean difference was 141.48 mm^(2)(P<0.05).There was significant difference in cyst one year after operation and the mean difference was-130.24 mm^(2)(P<0.05).During the following-up period,there were no cases of maxillofacial mucosal perforation in the cyst group and non-cystic group,no nasal bleeding,infection and maxillary sinusitis occurred,the wound healed well in the bone graft area,and the cumulative retention rate of implants was 100%.ConclusionThe lateral wall window lifting of maxillary sinus does not affect the natural development of cysts and maxillary sinus pseudocyst does not affect implant root osteogenesis.The operative method of implant implantation for the same period of maxillary sinus lateral wall fenestration is feasible.
