摘　　要：目的探讨重组人血管内皮抑制素注射液与含铂类化疗治疗晚期非小细胞肺癌胸腔积液急性发作的临床疗效。方法选取湘潭市第一人民医院2019年1月—2020年1月收治的晚期非小细胞肺癌胸腔积液急性发作患者88例,按照随机数字表法分为对照组43例和观察组45例。对照组给予含铂类方案化疗,观察组在对照组基础上给予重组人血管内皮抑制素注射液化疗。比较2组临床疗效,治疗前后胸腔积液血管内皮生长因子(VEGF)及低氧诱导因子1α(HIF-1α)水平,并观察2组毒副作用发生情况。结果观察组总有效率高于对照组(P<0.05)。治疗前2组胸腔积液VEGF、HIF-1α水平比较,差异无统计学意义(P>0.05);治疗后观察组VEGF、HIF-1α水平低于对照组(P<0.05)。观察组毒副作用发生率低于对照组(P<0.05)。结论重组人血管内皮抑制素注射液与含铂类化疗治疗晚期非小细胞肺癌胸腔积液急性发作的临床疗效确切,可降低患者胸腔积液VEGF及HIF-1α水平,且安全性较高。Objective To investigate the clinical efficacy of recombinant human endostatin injection and platinum based chemotherapy in the treatment of advanced non-small cell lung cancer with acute pleural effusion.Methods A total of 88 cases of patients with advanced non-small cell lung cancer with acute pleural effusion were selected from January 2019 to January 2020 in Xiangtan First People’s Hospital,which were divided into control group(43 cases)and observation group(45 cases)according to random number table.The control group was given platinum containing chemotherapy,and the observation group was given recombinant human endostatin injection chemotherapy on the basis of the control group.The clinical efficacy,the levels of vascular endothelial growth factor(VEGF)and hypoxia inducible factor-1α(HIF-1α)in pleural effusion before and after treatment,and the incidence of side effects were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).Before treatment,there was no significant difference in the levels of VEGF and HIF-1αin pleural effusion between the two groups(P>0.05);after treatment,the levels of VEGF and HIF-1αin the observation group were lower than those in the control group(P<0.05).The incidence of side effects in the observation group was lower than that in the control group(P<0.05).Conclusion Recombinant human endostatin injection and platinum based chemotherapy are effective in the treatment of advanced non-small cell lung cancer patients with acute pleural effusion.They can reduce the levels of VEGF and HIF-1αin pleural effusion,and with high safety.
