摘 要:目的探讨二维超声与超声造影在复发性肝细胞癌(HCC)介入微波消融治疗中的应用。方法选取2018年1月至2020年1月间空军军医大学西京医院收治的104例复发性HCC患者为研究对象,所有患者均采用介入微波消融治疗。按照采用的治疗方法不同进行分组,采用二维超声检查的52例患者纳入对照组,采用超声造影检查的52例患者纳入研究组。比较术后1个月时,两组患者的临床疗效。比较术后3d内,两组患者的术后并发症。比较两组患者的无瘤生存时间。结果术后1个月时,研究组患者肿瘤完全消融率为94.8%,高于对照组的83.5%,差异有统计学意义(P<0.05)。术后3d内,两组患者术后疼痛、发热和呕吐发生率比较,差异无统计学意义(P>0.05)。研究组患者无瘤生存时间为(11.32±10.84)个月,对照组为(12.62±10.41)个月,两组组间比较,差异无统计学意义(t=0.62,P<0.05)。结论超声造影应用于复发性HCC患者介入微波消融治疗中,能够提高肿瘤完全消融率,同时治疗安全性较高。Objective To explore the application of two-dimensional ultrasound versus contrast-enhanced ultrasound in the interventional microwave ablation of recurrent hepatocellular carcinoma(HCC).Methods A total of 104 patients with recurrent HCC were selected as the research subjects between January 2018 and January 2020,and they were classified into an experimental group and a control group according to the therapies they received,with 52 patients in each group.Patients receiving contrast-enhanced ultrasound examination were included in the experimental group and patients receiving two-dimensional ultrasound examination were included in the control group.The clinical efficacy was compared between the two groups at 1 month after the surgery.The incidence of postoperative complications was compared between the two groups at 3 d after the surgery.Moreover,tumor-free survival was compared between the two groups.Results At 1 month after the surgery,the complete ablation rate was 94.8%in experimental group which was significantly higher than 83.5%of the control group(P<0.05).There was no significant difference in the incidence of postoperative pain,fever and vomiting between the groups(P>0.05).Tumor-free survival was 11.32±10.84 months in experimental group and 12.62±10.41 months in control group(P>0.05).Conclusion Contrast-enhanced ultrasound can improve the complete ablation rate and ensure the safety of treatment in patients with recurrent HCC undergoing interventional microwave ablation.
