摘 要:目的探讨血清维生素D在原发性肺癌患者营养评估中的应用效果。方法选取2017年1月至2019年1月间上海市宝山区罗店医院收治的84例原发性肺癌患者,根据患者病情程度分为前期组(A组)、中期组(B组)和晚期组(C组),每组28例。对三组患者血清中的维生素D缺乏率和25羟维生素D水平进行检测。采用PG-SGA营养评估法对三组患者营养状况进行评估,比较患者的血清清蛋白(Alb)水平、淋巴细胞计数(TLC)、血清前清蛋白(PA)及血红蛋白(Hb)等指标。采用多元线性回归,以25羟维生素D水平为因变量,与Hb、PA、肌酐(Cr)、Alb、尿蛋白(UP)等指标的影响因素进行分析。结果A组患者的25羟维生素D水平低于B、C两组,处于维生素D缺乏状态。A、B、C三组的维生素D缺乏率分别为73.8%(维生素D缺乏)、62.4%(维生素D不足)和47.6%(维生素D充足)。A组患者的维生素D缺乏率低于B、C两组。A组患者的Alb、TLC、PA、Hb、Cr指标水平低于B、C两组,B组患者的上述指标水平均较C组更低,C组的UP水平较A、B两组低。25羟维生素D水平与Hb、PA、Cr和Alb水平呈正相关关系,与UP水平呈负相关关系。结论血清维生素D在原发性肺癌患者的营养评估中,可有效评估患者的营养状况,有很高的临床推广价值。Objective To study the significance of serum vitamin D for nutritional assessment in patients with primary lung cancer.Methods From January 2017 to January 2019,84 patients with primary lung cancer were selected at Luodian Hospital of Shanghai Baoshan District.Based on patients'condition,they were categorized into three groups:a prophase group(group A),a metaphase group(group B)and an anaphase group(group C).Each group had 28 patients.The serum vitamin D deficiency rate and 25 hydroxyvitamin D level were measured in the three groups.Nutritional status was evaluated in the three groups using patient generated subjective global assessment(PG-SGA)nutritional assessment method.Albumin(Alb),lymphocyte count(TLC),prealbumin(PA)and hemoglobin(Hb)were measured and compared.Multiple linear regression was used to analyze the association between 25 hydroxyvitamin D level and factors such as Hb,PA,creatinine(Cr),Alb and proteinuria(UP)with 25 hydroxyvitamin D using as a dependent variable.Result The level of 25 hydroxyvitamin D was significantly lower in group A than in group B and group C exhibiting vitamin D deficiency.The rate of vitamin D deficiency in group A,group B and group C was 73.8%(vitamin D deficiency),62.4%(vitamin D shortage)and 47.6%(vitamin D sufficiency),respectively.Obviously,the rate of vitamin D deficiency in group A was significantly lower than in group B and group C.The levels of indexes including Alb,TLC,PA,Hb and Cr were significant lower in group A than in the other two groups.The levels of above indicators were lower in group B than in group C,and the UP level is lower in group C than in the other two groups.The level of 25 hydroxyvitamin D is positively associated with the levels of Hb,PA,Cr and Alb,and negatively associated with the level of UP.Conclusion Measuring serum vitamin D can effectively assess the nutritional status in patients with primary lung cancer,which has a high clinical value.
