Twin-block与Forsus功能矫治器矫治骨性Ⅱ类错[牙合]畸形的效果比较

摘　　要：目的比较Twin-block与Forsus功能矫治器矫治骨性Ⅱ类错[牙合]畸形的效果。方法66例骨性Ⅱ类错[牙合]畸形患儿,随机分为对照组和观察组,各33例。对照组戴用Twin-block功能矫正器进行矫治,观察组戴用Forsus功能矫治器进行矫治。比较两组患儿治疗前后的上下齿槽座角(ANB)、下齿槽座角(SNB)、上齿槽座角(SNA)、上中切牙角(U1-SN)、下颌长度(Co-Gn)、下颌升支高度(Co-Go)、下中切牙角(L1-MP)及覆盖、覆合情况。结果治疗后,两组患儿的ANB、SNA、覆盖、覆合较治疗前减小,SNB、U1-SN、Co-Gn、Co-Go、L1-MP较治疗前增大,差异有统计学意义(P<0.05);治疗后,观察组的ANB(3.75±0.83)°、SNA(80.51±1.24)°、覆盖(3.05±0.77)mm、覆合(2.16±0.29)mm小于对照组的(4.31±1.02)°、(81.67±2.19)°、(5.35±1.21)mm、(2.47±0.42)mm,SNB(80.45±6.23)°、Co-Gn(115.29±3.9)mm大于对照组的(75.74±4.15)°、(111.73±2.3)mm,差异有统计学意义(P<0.05);但两组治疗后的U1-SN、Co-Go、L1-MP比较,差异均无统计学意义(P>0.05)。结论Forsus功能矫治器改善骨性Ⅱ类错[牙合]畸形的效果更好,更利于促下颌生长,下颌骨长度明显增长,值得临床推广。Objective To compare the effects of the Twin-block and Forsus functional appliance for skeletal classⅡmalocclusion patients.Methods A total of 66 children with skeletal classⅡmalocclusion were randomly divided into control group and observation group,with 33 cases in each group.The control group was treated with Twin-block functional appliance,and the observation group was treated with Forsus functional appliance.The A point-nasion-B(ANB),sella-nasion-B(SNB),sella-nasion-A(SNA),upper incisor-sn plane(U1-SN),Co-Gn,Co-Go,lower incisor-mandibular plan(L1-MP),covering and overlap were compared between the two groups.Results After treatment,the ANB,SNA,covering and overlap of the two groups were decreased compared with those before treatment,and SNB,U1-SN,Co-Gn,Co-Go and L1-MP were increased compared with those before treatment.All the difference was statistically significant(P<0.05).After treatment,the ANB(3.75±0.83)°,SNA(80.51±1.24)°,covering(3.05±0.77)mm and overlap(2.16±0.29)mm of the observation group were less than(4.31±1.02)°,(81.67±2.19)°,(5.35±1.21)mm and(2.47±0.42)mm of the control group,and SNB(80.45±6.23)°,Co-Gn(115.29±3.9)mm were more than(75.74±4.15)°and(111.73±2.3)mm of the control group.All the difference was statistically significant(P<0.05).but there was no statistically significant difference in U1-SN,Co-Go and L1-MP after treatment between the two groups(P>0.05).Conclusion Forsus functional appliance shows good effect in improving skeletal classⅡmalocclusion,which is more conducive to promoting the growth of mandible,and the length of mandible is significantly increased.It is worthy of clinical promotion.
