摘 要:目的:探讨CCL2和ETS-1在皮肤黑色素瘤组织中的表达及其临床意义。方法:选取2012年06月至2015年06月本院收治的皮肤黑色素瘤患者70例,取其黑色素瘤组织。另选取同期在本院外科手术切除的正常皮肤组织70例作为对照。采用免疫组化染色法检测CCL2和ETS-1蛋白表达,分析其与临床病理参数的关系。采用COX回归模型分析CCL2和ETS-1蛋白表达与皮肤黑色素瘤患者预后的关系。结果:皮肤黑色素瘤组织中CCL2、ETS-1阳性表达率明显高于正常皮肤组织(P<0.05)。CCL2和ETS-1表达与患者年龄、性别无关(P>0.05),与肿瘤大小、分化程度、浸润程度、TNM分期、淋巴结是否转移有关(P<0.05)。CCL2与ETS-1在皮肤黑色素瘤组织中的表达呈正相关(r=0.452,P<0.05)。CCL2、ETS-1阳性表达组3年存活率均显著低于阴性表达组(P<0.05)。COX回归分析显示,CCL2和ETS-1表达、基层浸润、淋巴结转移是影响皮肤黑色素瘤预后的独立危险因素(P<0.05)。结论:CCL2和ETS-1在皮肤黑色素瘤组织中均高表达,且与肿瘤分化程度、淋巴结转移等病理参数及预后有关,可能参与皮肤黑色素瘤发生发展。Objective:To investigate the expressions of CCL2 and ETS-1 in cutaneous melanoma and their clinical significances.Methods:70 patients with cutaneous melanoma admitted to our hospital from June 2012 to June 2015 were selected and their melanoma tissues were taken.Another 70 cases of normal skin tissue excision in our hospital during the same period were selected as control.Immunohistochemical staining was used to detect the expressions of CCL2 and ETS-1 proteins,and their relationships with clinicopathological parameters were analyzed.COX regression model was used to analyze the relationships between the expressions of CCL2 and ETS-1 proteins and the prognosis of patients with cutaneous melanoma.Results:The positive rates of CCL2 and ETS-1 in skin melanoma tissue were significantly higher than those in normal skin tissue(P<0.05).The expressions of CCL2 and ETS-1 were not correlated with age and gender of patients(P>0.05),but with tumor size,differentiation,invasion,TNM stage and lymph node metastasis(P<0.05).The expressions of CCL2 and ETS-1 in cutaneous melanoma were positively correlated(r=0.452,P<0.05).The 3-year survival rates in CCL2 and ETS-1 positive expression groups were significantly lower than those of negative expression groups(P<0.05).COX regression analysis showed that CCL2 and ETS-1 expressions,primary infiltration and lymph node metastasis were independent risk factors affecting the prognosis of cutaneous melanoma(P<0.05).Conclusion:CCL2 and ETS-1 are highly expressed in cutaneous melanoma tissues,which are related to the pathological parameters of the degree of differentiation,lymph node metastasis and prognosis,and may be involved in the occurrence and development of cutaneous melanoma.
