胃间质瘤与非胃间质瘤的超声表现及病理危险度分级的对比研究

摘　　要：目的:比较胃间质瘤与非胃间质瘤的超声图像表现及病理危险度分级间的差异,评价超声检查预测胃间质瘤和非胃间质瘤危险度的临床价值。方法:回顾性分析经手术后病理证实为胃肠道间质瘤(GIST)的55例患者的超声图像表现,按肿瘤的生长部位分为胃间质瘤组和非胃间质瘤组,对比分析两组间超声图像表现及病理危险度分级的差异。结果:非胃间质瘤组的恶性危险度更高,在瘤体最大径≤5 cm、呈类圆形、内部不伴无回声、血流稀少方面,两组间危险度相比较均具有统计学差异(P<0.05)。同时,瘤体最大径>5 cm、形态不规则、内伴无回声、血流丰富和伴有转移的GIST恶性危险度更高,具有统计学差异(P<0.05)。结论:胃间质瘤与非胃间质瘤的恶性程度存在一定差异,非胃来源的GIST恶性程度高于胃来源的间质瘤,超声图像特征有助于评估肿瘤的危险度,为临床治疗和判断预后提供指导信息。Objective:To estimate clinical value of ultrasound in calculating pathological grading of gastric stromal tumor and extragastric stromal tumor by comparing the difference of their ultrasonic features and pathological grading.Methods:The ultrasonic features of 55 cases with gastrointestinal stromal tumor(GIST)confirmed by postoperative pathology were retrospectively studied.All the cases were divided into gastric stromal tumor group and extragastric stromal tumor group.The difference of the ultrasonic features and pathological grading of the two groups was comparatively analysed.Results:The malignant risk of extragastric stromal tumor was higher than that of gastric stromal tumor when the maximum diameter of tumor was≤5 cm,quasi-circular,without anecho,with rare blood flow signal,and there was statistical difference(P<0.05).GIST with the maximum diameter of tumor>5 cm,irregular shape,with anecho,with rich blood flow signal and with metastasis had higher risk of malignant degree,and there was statistical difference(P<0.05).Conclusion:There was difference between the pathological grading of gastric stromal tumor and that of extragastric stromal tumor,and the malignant risk of extragastric stromal tumor was higher than that of gastric stromal tumor.The ultrasonic features conduce to assessing pathological grading of tumor and can provide instructional information for clinical treatment and judging the prognosis.
