摘 要:目的:探讨细胞紧密连接蛋白1(CLDN1)和Ⅱ型跨膜丝氨酸蛋白酶4(TMPRSS4)在肝癌中的表达及其临床意义。方法:选取手术前未行放化疗的临床肝癌组织标本89例,并选取55例非肝癌组织标本作为对照组,通过免疫组化染色法检测CLDN1与TMPRSS4的表达水平,并且分析CLDN1和TMPRSS4表达与患者临床病理特征和预后的关系。结果:CLDN1在肝癌组织中的表达阳性率为65.2%,而在非肝癌组织中的表达阳性率为25.5%,差异具有统计学意义(P<0.001);TMPRSS4在肝癌组织中的表达阳性率为73.0%,而在非肝癌组织中的表达阳性率为30.9%,差异具有统计学意义(P<0.001)。CLDN1和TMPRSS4的表达与患者肿瘤分化程度、TNM分期相关。Spearman相关性分析提示TMPRSS4与CLDN1表达水平呈正相关。CLDN1和TMPRSS4高表达组生存率均低于CLDN1和TMPRSS4低表达组,差异具有统计学意义(P<0.05)。结论:CLDN1与TMPRSS4在肝癌的发生、发展过程中可能起着协同作用,联合检测CLDN1与TMPRSS4可能对判断肝癌的预后具有一定指导意义。Objective:To investigate the expression of CLDN1 and TMPRSS4 in hepatocellular carcinoma(HCC)and their clinical significance.Methods:89 specimens of HCC tissues without radiation and chemotherapy before operation were collected as observation group and 55 specimens of non-tumor tissues were collected as control group.Immunohistochemistry staining was used to explore the expression of CLDN1 and TMPRSS4 in two groups and the correlation between CLDN1 and TMPRSS4 expression and clinicopathological features and prognosis was analysed.Results:The positive expression rate of CLDN1 in non-tumor tissues was 25.5%,much more lower than that of HCC tissues(65.2%)(P<0.001).TMPRSS4 expression rate of HCC tissues was 73.0%,while that of non-tumor tissues was 30.9%,and the difference was statistically significant(P<0.001).In addition,the expression of CLDN1 and TMPRSS4 was correlated with tumor differentiation degree and TNM staging(P<0.05).There was an positive relation between the expression of TMPRSS4 and CLDN1 in HCC tissues.The survival rate of CLDN1 and TMPRSS4 high expression group was lower than those low expression group(P<0.05).Conclusion:CLDN1 may cooperate with TMPRSS4 in development in HCC.The combined detection of CLDN1 and TMPRSS4 may have certain guiding significance for judging the prognosis of HCC.
