摘 要:目的探讨微钛板内固定椎管重建术在儿童椎管内肿瘤切除术中的应用效果。方法回顾性分析2011-01—2014-01在郑州大学附属儿童医院行全椎板切开后应用两孔微钛板内固定椎管重建的椎管内肿瘤患儿的临床资料。采用改良McCormick分级标准评估临床症状,应用影像学检查评估脊柱的稳定性。结果43例患儿中,畸胎瘤10例,皮样囊肿10例,表皮样囊肿9例,蛛网膜囊肿8例,肠源性囊肿2例,室管膜瘤2例,星形细胞瘤2例。其中39例(90.7%)完全切除,4例(9.3%)部分切除。术后随访6~24个月,椎板成形术部位均完全愈合,临床症状明显改善。影像学检查显示无脊柱失稳。结论微钛板内固定椎管重建术,既可以提供一个合适的手术视野,又可重建脊柱稳定性,是一种安全、有效的手术方式。Objective To explore the reconstruction strategy and therapeutic effect of micro titanium plate internal fixation for spinal canal reconstruction in children with intraspinal tumor resection.Methods From January 2011 to January 2014,a total of 43 children with intraspinal tumors diagnosed in the Department of Neurosurgery,Affiliated Children’s Hospital of Zhengzhou University,were enrolled in this study.The patients were treated with total laminectomy and titanium plate internal fixation,Age 1.2~7.8 years old.The modified McCormick classification standard was used to evaluate the clinical symptoms,and imaging was used to evaluate the spinal stability.Results Among the 43 cases,10 cases were teratoma,10 cases were dermoid cyst,9 cases were epidermoid cyst,8 cases were arachnoid cyst,2 cases were enterogenic cyst,2 cases were ependymoma,2 cases were astrocytoma,39 cases(90.7%)were completely removed,4 cases(9.3%)were partially removed.All patients were followed up for 6 to 24 months.All patients healed completely and the clinical symptoms improved significantly.Imaging follow-up showed no spinal instability.Conclusion Micro titanium plate internal fixation for spinal canal reconstruction can not only provide a suitable surgical field,but also reconstruct the stability of the spine,which is a safe and effective surgical method.
