高龄膀胱癌合并良性前列腺增生患者的同期电切治疗的临床效果分析

摘　　要：目的探讨高龄膀胱癌合并良性前列腺增生(BPH)患者的同期电切治疗的可行性及疗效。方法回顾性分析2013年1月至2017年1月本院收治的98例高龄膀胱癌合并BPH患者的临床资料,根据治疗情况分为两组,其中46例同期行经尿道膀胱肿瘤电切术(TURBT)联合前列腺电切术(TURP)患者纳人联合组,52例单纯行TURBT患者纳人对照组。比较两组患者的术中情况、术后并发症及膀胱癌复发率。结果联合组患者的手术时间、术中出血量均明显高于对照组,差异具有统计学意义(P<0.05),而两组的留置导尿管时间、住院时间、术后膀胱出血率比较,差异无明显统计学意义(P>0.05)。两组术后的并发症发生率、膀胱肿瘤复发率、多次复发率、前列腺窝复发率比较,差异无统计学意义(P>0.05)。结论同期行TURBT联合TURP治疗高龄膀胱癌合并BPH是一种安全、有效的治疗方法,不增加肿瘤复发风险及前列腺窝种植的风险,值得临床推广。Objective To investigate the safe and effective management of bladder cancer combined with benign prostatie hyperplasia(BPH)in elderly patients.Methods We analyzed retro-spectively 98 old patients of bladder cancer accompanied with BPH from January 2013 to January 2017.Forty-six cases who underwent simultaneous transurethral resection of bladder cancer(TURBT)and transurethral resection of prostate(TURP)were classified as combined treatment group.Fifty-two cases who underwent TURP were classified as control group.Comparison between the two groups was conducted in terms of intraoperative and postoperative conditions and recurrence rate of bladder cancer.Results The operation time,blood loss and IPSS score were lower in the control group than those in the combined treatment group(P<0.05).No significant difference was found between two groups in terms to patient age,tumor stage,tumor size,hospital stay,catheter retention time,postoperation com-plication rate and recurrence rate(P>0.05).Conclusions Simultancous transurethral electrocision of bladder and prostate is safe and effective to treat bladder cancer combined with BPH in elderly pa-tients without increasing recurrence rate of bladder cancer.
