摘 要:目的 探讨尿修饰核苷作为宫颈鳞癌分子标志物的可行性。方法 利用超高效液相色谱-串联质谱(UPLCMS)技术检测55例宫颈鳞癌患者和60例健康人群15种尿修饰核苷含量,采用偏最小二乘法-判别分析(PLS-DA)、多因素回归分析及ROC曲线,探讨尿修饰核苷与宫颈鳞癌临床特征的关系。结果 15种修饰核苷线性方程在5 ng/ml~800 ng/ml(Pseu在1μg/ml~12μg/ml)浓度内相关系数良好,检出限均<1. 0 ng/ml(Pseu <10. 0 ng/ml),回收率为83. 92%~127. 99%,重复性的相对标准偏差(RSD)为5. 97%~9. 45%。其中3种尿修饰核苷(Pseu、N4C、NNdm G)在研究组中的水平显著高于对照组(P <0. 05),并与FIGO分期相关(P <0. 05),Pseu、NNdm G、N4C、联合检测ROC-AUC值分别为0. 873、0. 927、0. 947、0. 951,联合检测时敏感度为93. 3%,特异度为93. 2%。结论 尿修饰核苷可区分宫颈鳞癌与健康人群,并与FIGO分期密切相关,可能成为筛查宫颈鳞癌的分子标志物。Objective To explore the feasibility of urinary modified nucleoside as a molecular marker of cervical squamous cell carcinoma.Methods A total of 15 urinary modified nucleosides in 55 patients with cervical squamous cell carcinoma and 60 healthy people were detected by ultrahigh performance Liquid chromatography tandem mass spectrometry(UPLC-MS).The relationship between urinary modified nucleosides and clinical characteristics of cervical squamous cell carcinoma was studied by PLS-DA,multivariate regression analysis and ROC curve.Results The 15 modified nucleosides calibrators showed a linear response from 5 ng/ml to 800 ng/ml(Pseu=1μg/ml-12μg/ml).The detection limit was less than 1.0 ng/ml(Pseu<10.0 n g/m l).The recovery rate was within 83.92%-127.99%,and the repetitive standard deviation(RSD)was within 5.97%-9.45%.The levels of 3 urinary modified nucleosides(P seu,N4C,NNdmG)in the study group were significantly higher than those in the control group(P<0.05),and further analysis of the pathological characteristics with cervical squamous cancer showed that the pathological characteristics had correlation with FIGO staging(P<0.05).The ROC-AUC values of Pseu,NNdmG,N4C and combined detection were 0.873,0.927,0.947,0.951,respectively.The sensitivity and specificity of combined detection were 93.3%and 93.2%.Conclusion Urinary modified nucleoside can distinguish cervical squamous cell carcinoma from healthy people,and is closely related to FIGO stage,which is expected to be a molecular marker for cervical squamous cell carcinoma screening.
