外周血CA125、septin-9表达与卵巢癌淋巴结转移的相关性

摘　　要：目的探讨外周血糖类抗原-125(CA125)、septin-9表达与卵巢癌淋巴结转移的相关性。方法抽取昌吉州人民医院2017年2月至2020年5月收治的62例卵巢癌患者为研究对象,回顾性分析其临床资料。根据术前穿刺活检结果,将淋巴结转移患者纳入转移组,无淋巴结转移患者纳入未转移组。比较两组基线资料与生化指标,分析外周血CA125、septin-9表达与卵巢癌淋巴结转移的相关性。结果纳入的62例卵巢癌患者中,14例出现淋巴结转移。两组年龄、病理类型、分化程度比较差异未见统计学意义(P>0.05)。两组CA125、septin-9 mRNA表达量比较差异有统计学意义(P<0.05)。经Logistic回归分析显示,外周血CA125、septin-9表达与卵巢癌淋巴结转移有关(P<0.05)。绘制受试者工作特征曲线结果显示,外周血CA125预测卵巢癌淋巴结转移的曲线下面积为0.795,预测价值中等;外周血septin-9预测卵巢癌淋巴结转移的曲线下面积为0.817,预测价值中等;外周血CA125与septin-9联合预测卵巢癌淋巴结转移的曲线下面积为0.909,预测价值较高。结论外周血CA125、septin-9表达与卵巢癌淋巴结转移有关,CA125、septin-9高表达可增加淋巴结转移风险,检测外周血CA125、septin-9水平可预测淋巴结转移风险。Objective To investigate the relationship of expression of carbohydrate antigen 125(CA125)and septin-9 with lymph node metastasis of ovarian cancer.Methods A total of 62 patients with ovarian cancer treated in Changji People’s Hospital from February 2017 to May 2020 were selected as research objects,and their clinical data were retrospectively analyzed.According to results of needle biopsy,patients with lymph node metastasis were included in the metastasis group,and patients without lymph node metastasis were included in the non metastasis group.The general data and biochemical indicators of the two groups were compared.The correlation of expression of CA125 and septin-9 in peripheral blood with lymph node metastasis of ovarian cancer was analyzed.Results Among 62 ovarian cancer patients in the study,14 cases had lymph node metastasis.There was no significant difference in age,pathological type,and differentiation degree between the two groups(P>0.05).However,there was a significant difference in the expression of CA125 and septin-9 mRNA between the two groups(P<0.05).Logistic regression analysis showed that expression of CA125 and septin-9 in peripheral blood was associated with lymph node metastasis of ovarian cancer(P<0.05).Results of receiver operating characteristic curve showed that area under curve(AUC)of peripheral blood CA125 in predicting lymph node metastasis of ovarian cancer was 0.795,with medium predictive value;the AUC of peripheral blood septin-9 in predicting lymph node metastasis of ovarian cancer was 0.817,with medium predictive value;and the AUC of peripheral blood CA125 combined with septin-9 in predicting lymph node metastasis of ovarian cancer was 0.909,with high predictive value.Conclusions The expression of CA125 and septin-9 in peripheral blood is related to lymph node metastasis of ovarian cancer.The high expression of CA125 and septin-9 can increase the risk of lymph node metastasis.Detecting CA125 and septin-9 in peripheral blood can predict the risk of lymph node metastasis.
