摘　　要：目的探讨E-钙黏蛋白、Ki-67、A20在浸润性乳腺癌患者中的表达及其临床价值。方法选取浸润性乳腺癌患者73例,检测治疗1个月后E-钙黏蛋白、Ki-67、A20在患者乳腺癌组织和癌旁组织中的阳性表达水平,分析其与患者临床症状间的相关性,对可能影响患者预后的相关因素进行单因素和多因素Cox分析。结果(1)Ki-67和A20在乳腺癌组织中的阳性表达显著高于癌旁组织,E-钙黏蛋白在乳腺癌组织中的阳性表达明显低于癌旁组织(P<0.05)。(2)研究结果显示,患者E-钙黏蛋白阳性表达与乳腺癌组织学分级、淋巴结转移有关(P<0.05),而与患者的年龄及肿瘤直径均无关(P>0.05);Ki-67阳性表达与患者组织学分级有关(P<0.05),但与患者年龄、淋巴结转移和肿瘤直径无关(P<0.05);A20阳性表达与患者肿瘤直径有关(P<0.05),但与年龄、淋巴结转移及组织学分级无关(P>0.05)。(3)截止随访终点,73例共有21例患者死亡,经比较死亡患者与生存患者之间E-钙黏蛋白、Ki-67、A20阳性率比较差异具有统计学意义(P<0.05)。结论E-钙黏蛋白高表达和Ki-67、A20低表达的浸润性乳腺癌患者预后较好,E-钙黏蛋白、Ki-67、A20对浸润性乳腺癌患者预后有一定的临床预测价值。Objective To explore the expression and clinical value of E-cadherin,Ki-67,A20 in patients with invasive breast cancer.Methods 73 cases of patients with invasive breast cance were selected.The positive expression levels of E-cadherin,Ki-67,and A20 in breast cancer tissues and adjacent tissues of patients were measured 1 month after treatment and their correlation with clinical symptoms was analyzed.Univariate and multivariate Cox analysis were performed on relevant factors that may affect patient outcomes,and independent risk factors that may influence patient outcomes were analyzed.Results(1)The positive expression of Ki-67 and A20 in breast cancer tissues was significantly higher than that in adjacent tissues.The positive expression of cadherin in breast cancer tissues was significantly lower than that in adjacent tissues(P<0.05).(2)The results of the study showed that the positive expression of E-cadherin in patients was related to the histological grade of breast cancer and lymph node metastasis(P<0.05),but was not related to the patient's age and tumor diameter(P>0.05);Ki-67 The positive expression is related to the patient’s histological grade(P<0.05),but has nothing to do with the patient’s age,lymph node metastasis and tumor diameter(P>0.05);the positive expression of A20 is related to the patient’s tumor diameter(P<0.05),but with age and lymph node the metastasis and histological grade were irrelevant(P>0.05).(3)By the end of the follow-up,21 patients died in 73 patients.After comparing the positive rates of E-cadherin,Ki-67 and A20 between dead and surviving patients,the difference was statistically significant(P<0.05).Conclusion Patients with invasive breast cancer with high expression of E-cadherin and low expression of Ki-67 and A20 have a better prognosis E-cadherin,Ki-67 and A20 are effective for patients with invasive breast cancer.Annual treatment prognosis has certain clinical predictive value.
