胸腔镜lvor-lewis与传统开胸sweet手术治疗胸中下段食管鳞状细胞癌的临床效果比较

摘　　要：目的比较胸腔镜lvor-lewis与传统开胸sweet手术治疗胸中下段食管鳞状细胞癌的临床效果。方法选取160例胸中下段食管鳞状细胞癌患者作为研究对象,随机将其分为2组。研究组80例,给予胸腔镜Ivor-Lewis手术治疗;对照组80例,给予Sweet手术治疗。观察2组患者手术指标、淋巴结清扫及转移情况、术后并发症发生率、治疗后的生活质量评分情况。结果①研究组患者插管时间、术后引流量、住院时间显著低于对照组(P<0.05);②研究组胸腔淋巴结及腹腔淋巴结的清扫个数多于对照组、转移个数少于对照组(P<0.05);③研究组术后并发症发生率显著低于对照组(P<0.05);④2组患者治疗后的生活质量评分情况比较,研究组显著高于对照组(P<0.05)。结论胸腔镜lvor-lewis手术治疗胸中下段食管鳞状细胞癌能有效改善患者的手术指标,缩短住院时间,减少并发症的发生率,提高患者的生活质量,具有积极的临床意义,值得推广和应用。Objective To compare the clinical effects of thoracoscopic lvor-lewis and traditional open-thoracic sweet surgery for esophageal squamous cell carcinoma.Methods 160 patients with esophageal squamous cell carcinoma of the middle and lower thorax were selected as the research object and were randomly divided into 2 groups,80 cases in the study group were treated with thoracoscopic lvor-lewis operation,and 80 cases in the control group were treated with sweet operation.The operation indexes,lymph node dissection and metastasis,incidence of postoperative complications and quality of life score after treatment were observed in the 2 groups.Results 1)The intubation time,postoperative drainage volume and hospitalization time of the study group were significantly better than those of the control group(P<0.05);2)The number of dissections of the thoracic and abdominal lymph nodes in the study group was more than that of the control group,and the number of metastases was less than that of the control group(P<0.05);3)The incidence of postoperative complications in the study group was significantly lower than that in the control group(P<0.05);4)After treatment,the quality of life score of the two groups was significantly higher than that of the control group(P<0.05).Conclusion Thoracoscopic lvor-lewis surgery in the treatment of middle and lower thoracic esophageal squamous cell carcinoma can effectively improve the operation index,shorten the hospitalization time,reduce the incidence of complications,and improve the quality of life of patients.It has positive clinical significance and is worthy of promotion and application.
