血清糖类抗原242水平在不同分期胰腺癌中的差异及其与患者生存期的相关性

摘　　要：目的探讨血清糖类抗原242(CA242)水平在不同分期胰腺癌中的差异及其与患者生存期的相关性。方法收集80例胰腺癌患者为研究对象纳入胰腺癌组,选取同期治疗的胰腺炎患者50例为良性病变组,另选同期进行健康检查的健康受试者50例为对照组。采用酶联免疫吸附试验法对3组受试者的CA242水平进行检测,并进行组间对比。对不同分期胰腺癌患者的CA242进行统计对比,并随访观察,统计对比不同分期患者的生存期,包括无进展生存期(PFS)、总体生存期(OS)。以Pearson检验对胰腺癌患者的CA242与生存期指标相关性进行分析探讨。结果胰腺癌组患者的CA242水平显著高于良性病变组且高于对照组,3组间差异有统计学意义(P<0.05)。胰腺癌组患者CA242水平随着病理分期的升高呈依次升高的趋势,而PFS、OS随着病理分期的升高而依次递减(P<0.05)。经Pearson检验分析,胰腺癌患者的CA242水平与生存期指标PFS、OS水平呈负相关性(P<0.05)。结论CA242在胰腺癌患者人群中呈现高表达,且在不同分期患者中水平有差异性。CA242水平与患者的生存期呈负相关性。对胰腺癌患者CA242水平的检测,对于胰腺癌的诊断、病情评估、疗效判断及预后生存期预测有重要临床价值。Objective To study the difference of serum carbohydrate antigen 242(CA242)level in patients with different stages of pancreatic cancer and its correlation with survival time.Methods A total of 80 patients with pancreatic cancer were collected as research subjects and included in the pancreatic cancer group.50 patients with pancreatitis treated at the same time were selected as the benign lesion group,and 50 healthy subjects who underwent health examinations at the same time were selected as the control group.Enzyme-linked immunosorbent assay was used to detect the CA242 levels of the 3 groups of subjects,and CA242 of patients with different stages of pancreatic cancer was counted and compared,followed up and observed,and the survival time of patients with different stages was statistically compared,including progression free survival(PFS)and overall survival(OS)were statistically compared.Pearson test was used to analyze the correlation between CA242 and survival index in patients with pancreatic cancer.Results The level of CA242 in pancreatic cancer group was significantly higher than that in benign lesion group and the control group(P<0.05).The level of CA242 in patients with pancreatic cancer showed a trend of increasing with the increase of pathological stage,while PFS and OS decreased with the increase of pathological stage(P<0.05).Pearson test showed that CA242 level was negatively correlated with PFS and OS in patients with pancreatic cancer(P<0.05).ConclusionCA242 is highly expressed in patients with pancreatic cancer,and the level is different in different stages of patients,and CA242 levels are negatively correlated with patients’survival.The detection of CA242 level in patients with pancreatic cancer has important clinical value for the diagnosis,disease evaluation,curative effect judgment and prognosis and survival prediction of pancreatic cancer.
