摘　　要：目的:评估牙周炎患者基础治疗后进行手术对残留牙周袋的长期影响。方法:回顾慢性牙周炎患者34例基础治疗后进行牙周手术,随访4年以上,随访间隔2~2.5年。记录残留牙周袋患牙的临床及影像学检查指标,与未手术治疗组39例比较分析其变化差异。结果:手术治疗组5~5.5年后,与未手术治疗组相比,定期维护后手术治疗组患者年缺失牙率更低(0.05∶0.11),牙周探诊深度(0.55∶0.39 mm)及附着丧失水平(1.53∶1.33 mm)的改善程度更好,骨根比变化更小(0.23%∶1.15%),PD>6 mm牙槽骨位点比例更少(1.19%∶2.35%)。结论:基础治疗后,手术治疗特别是对于PD>6 mm的患者,能够更好地保持牙周治疗的长期疗效。Objective:To evaluate the long-term effects of surgical treatment of the residual periodontal pocket after initial therapy and supportive therapy.Methods:Modified widman flap surgery were used to treate 34 patients with residual deep pockets after initial periodontal therapy.After treatment the patients were followed up for more than 4 years with a interval of 2-2.5 yeras.The periodontal indexes and imaging findings were recorded,analyzed and compared with those of 39 patients underwent non-surgical treatment(controls).Results:After an average of 5-5.5 years maintenance period,patients in the surgical group showed lower tooth loss rate(0.05 vs 0.11);better improvement of PD and CAL(PD:0.55 mm vs 0.39 mm;CAL:1.53 mm vs 1.33 mm);lower rate of sites with greater alveolar bone reduction(0.23%vs 1.15%)and lower reduction of alveolar bone in the sites of residual deep pockets with PD>6 mm(1.19%vs 2.35%).Conclusion:Surgical treatments is effective in keeping periodontal health for long-term especially for the patients with the residual periodontal pocket of PD>6 mm.
