摘 要:目的:研究青岛市5岁重度低龄龋病儿童和健康儿童龈上菌斑的微生物结构及其差异。方法:纳入34例健康(H组)和31例重度龋病(C组)5岁儿童,采集龈上菌斑样本共65份,利用Illumina MiSeq测序平台对16S rRNA V3-V4区进行双端测序,分析微生物群落结构和多样性,并建立龋病风险评估模型。结果:C组的物种多样性指数(Shannon和Simpson)高于H组,2组菌群结构均存在显著差异(P<0.05),C组菌群结构更复杂,H组菌群结构比C组更为集中;鉴别出H组高表达的健康相关微生物(P<0.05)及C组高表达的可疑致龋微生物(P<0.05),如变形链球菌;基于龈上菌斑种水平微生物图谱建立的龋病风险评估模型辨别健康和龋病者的准确率可达71%。结论:龈上菌斑菌群和特定细菌种类的检测有助于评估和预测低龄儿童龋病风险。Objective:To compare the microbiota of supragingival plaque in 5-year-old healthy children and in those with severe dental caries in Qingdao.Methods:65 samples of supragingival plaque were collected from 34 healthy children(H group)and 31 children with severe dental caries(C group).The bacterial composition was determined by the Illumina MiSeq DNA sequencing of 16S rRNA gene.The microbiotas associated with dental caries were identified,and the caries risk assessment models were established.Results:The microbial diversity(Shannon and Simpson)of C group was greatr in C group than that in H group(P<0.05),and community structure in the supragingival plaque microbiota was different between the 2 groups(P<0.05),that of group C was more complex.More convergent microbial community structure was observed in H group compared with C group(P<0.001).Health-enriched and caries-enriched taxa were identified,e.g.,Streptococcus mutans.The model for caries risk assessment based on the species-level bacterial profiles reached up to 71%accuracy.Conclusion:Predictive model based on supragingival plaque microbiota can be served as a valuable approach for screening and assessing the children's risk of caries development.
