高血压患者笑气吸入条件下Er∶YAG激光辅助牙周基础治疗的临床研究

摘　　要：目的:比较高血压患者在笑气(N_(2)O)吸入条件下Er∶YAG激光辅助牙周基础治疗与传统牙周基础治疗的临床效果。方法:28例高血压患者牙列的四个象限分次随机接受4种牙周基础治疗:(1)局麻下手工龈下刮治根面平整(scalingandrootplaning,SRP);(2)N_(2)O吸入下SRP;(3)局麻下,Er∶YAG激光+手工SRP;(4)N_(2)O吸入下,Er∶YAG激光+手工SRP。治疗中记录血压、疼痛VAS评分与牙周临床指标。使用SPSS 22.0进行统计学分析。结果:激光辅助牙周治疗的两组牙周临床指标改善值显著高于局麻+SRP组(P<0.05)。N_(2)O吸入2组VAS评分均显著低于局麻下治疗的2组,其中N_(2)O+激光+SRP组VAS评分最低(P<0.05)。麻醉时N_(2)O吸入2组的血压值低于局麻2组的血压值(P<0.05)。结论:N_(2)O吸入联合Er∶YAG激光治疗较传统牙周基础治疗临床效果好、更舒适,并可能有助于维持血压的稳定。Objective:To study the clinical efficacy of nitrous oxide(N_(2)O)inhalation in periodontal inital therapy with Er∶YAG laser for patients with hypertension.Methods:28 patients were randomly assigned into 4 groups by split-mouth design:(1)Traditional subgingival scaling and root planing(SRP);(2)SRP with N_(2)O inhalation anesthesia;(3)Er∶YAG laser+SRP with local anesthesia;(4)Er∶YAG laser+SRP with N_(2)O inhalation anesthesia.The blood pressure and the pain VAS score were recorded,and the periodontal clinical indicators were assessed at the baseline,4 weeks and 3 months after treatment.The periodontal indexes,pain VAS and blood pressure were compared among the 4 groups.The data was statistically analyzed.Results:4 weeks and 3 months after treatment,PD,GI and BI of the 2 laser SRP groups were significantly lower than those of traditional SRP groups(P<0.05).The VAS of the 2 N_(2)O inhalation groups were significantly lower than those the local anesthesia groups;the VAS of N_(2)O+laser SRP group was the lowest(P<0.05).At the stage of anesthesia the blood pressure of the 2 N_(2)O inhalation groups was lower than that of the 2 local anesthesia groups(P<0.05).Conclusion:Compared with traditional periodontal treatment,N_(2)O inhalation combined with Er∶YAG laser+SRP may reduce the discomfort and obtain better clinical effects for patients with periondontal desease and hepertension;in addition,the patient's blood pressure may more stable during treatment.
