摘　　要：目的探讨磁共振检查在Marchiafava-Bignami病诊断中的应用价值。方法选取并分析我院收治的48例Marchiafava-Bignami病患者的临床资料及头颅MRI表现。结果48例患者中急性型26例,亚急性型8例,慢性型14例。所有患者均有长期酗酒史。胼胝体内病灶为左右对称分布。急性型及亚急性型患者病变区胼胝体体积正常或稍肿胀,病灶呈T1WI稍低信号,T2WI、Flair及DWI高信号。慢性型病变区胼胝体萎缩,病灶内可有囊变呈T1WI低信号,T2WI高信号,Flair及DWI低信号,矢状位上表现为典型的“三明治”征象。胼胝体外病灶中,41例合并皮层下脑白质变性,22例累及小脑中脚,14例累及前联合,3例累及内囊,1例累及岛叶皮质。5例患者经临床治疗后复查头颅MRI,其中2例颅内病灶较前吸收,1例病灶内见少量出血。结论Marchiafava-Bignami病的临床表现多样且无特异性,而MRI检查能准确、及时地发现病灶,其影像学表现具有特征性,对于Marchiafava-Bignami病的诊断及疗效评价具有重要价值。Objective To explore the value of MRI in the diagnosis of marchiafava bignami disease.Methods The clinical data and MRI findings of 48 patients with Marchiafava Bignami disease were analyzed retrospectively.Results Among the 48 patients,26 were acute type,8 were subacute type and 14 were chronic type.All patients had a long history of alcohol abuse.The lesions in the corpus callosum were symmetrical.The volume of corpus callosum was normal or swollen in the lesion area of patients with acute and subacute type,the lesions showed low signal on T1WI and high signal on T2WI,flair and DWI.In chronic lesions,the corpus callosum was atrophic.There were cystic lesions with low signal on T1WI,high signal on T2WI,low signal on flair and DWI,and it showed typical"sandwich"sign on sagittal position.Among the lesions outside the corpus callosum,41 cases combined with subcortical leukoencephalopathy,22 cases involved in middle cerebellar peduncle,14 cases in anterior syndesmosis,3 cases in internal capsule,and 1 case in insular cortex.After clinical treatment,5 patients were reexamined with MRI,2 of whom were better than before,1 case presented with small amount of bleeding in the focus.Conclusion The clinical manifestations of Marchiafava Bignami disease are various and nonspecific,while MRI can accurately and timely find the lesions,and its imaging manifestations are characteristic,which is of great value for the diagnosis and efficacy evaluation of Marchiafava Bignami disease.
