摘　　要：目的探讨鼻型结外NK/T细胞淋巴瘤(NKTCL)的影像学表现,以提高诊断的准确性。方法选取并分析经病理证实的24例鼻型NKTCL的头颈部MRI及其中3例胸部CT资料。结果24例中肿瘤发生于鼻腔17例,均累及单侧鼻腔,局限于单侧鼻腔6例、另11例见鼻腔外多个部位受侵犯;发生于鼻腔外7例,鼻咽受侵3例、口咽受侵2例、鼻咽及口咽均受侵者2例。11例患者见27处骨质破坏区,4例鼻中隔大穿孔,2例合并硬腭穿孔,6例淋巴结增大。17例在T1WI及T2WI呈等信号,3例见小斑片状T1WI低信号及T2WI高信号。增强后15例表现为轻中度均匀强化;5例见小斑片状坏死无强化区;另4例T1WI、T2WI及增强扫描均未发现软组织肿块影。3例合并肺部异常,其中2例表现为两肺多发节段性实变影,部分实变影内见空气支气管征,另1例两肺见弥漫性粟粒状结节。结论鼻型NKTCL的MRI表现具有一定的特征性,部分病例可合并肺部异常。Objective To analyse the imaging findings of extranodal natural killer/T-Cell lymphoma of nasal type.Methods24 pathologically confirmed nasal NK/T-cell lymphoma patients were included in this study in which MR images in the head and neck region and 3 cases CT data of the lung were analysed retrospectively.Results 17 cases originated from nasal cavity,all involed unilateral nasal cavity,6 cases were located in the unilateral nasal cavity,the other 11 cases were seen several areas outside the nasal cavity involvement,7 cases originateed from extra nasal cavity,including nasopharynx in 3 cases,oropharynx in 2 cases,both nasopharynx and oropharynx in 2 cases.Bone destruction was observed in 11 patients with 27 bones involved.4 cases was found perforation of nasal septum,combination with palate perforation was found in 2 cases.Lymph node enlargement was detected in 6 cases.On MRI,17 cases showed isointensity on T1WI and T2WI,3 cases patchy of T1WI low signal,and T2WI high signal intensity was found,15 cases showed mild to moderate homogenous enhancement,5 patients were seen little patchy necrosis without reinforced region.4 cases were negative both on T1WI and T2WI images.Abnormality of the lung was found in 3 cases,two cases showed two pulmonary multiple segmental consolidation,air bronchogram was seen in some lesions,another cases showed numerous miliary nodules in diffuse distribution in both lungs.Conclusion Definite features were demonstrated on MRI images of extranodal natural killer/T-Cell lymphoma of nasal type and abnormality of the lung could be found in some cases.
