摘　　要：目的探讨结构式报告在胃癌增强CT术前分期中的可行性及临床应用价值.方法回顾性入组30例胃癌术前分期增强CT检查,入组病例均包括1份自由文本报告和1份结构式报告.对结构式报告与自由文本报告的内容进行评价.另外由11名影像科医师对报告的便利度和教学度进行评价;2名胃肠外科医师对报告进行满意度评价,包括主观感受评价和信息完整评价.评价采用非参检验进行统计分析.结果报告一致性好的描述内容包括病变部位、强化特点、脏器转移、浆膜层情况、淋巴结的评估及TNM分期;一致性差的包括胃整体评估、血管评估、病变的密度特点,一致性一般的包括病变范围.影像科医师对结构式报告的主观感受均高于自由文本报告(P<0.05),外科医师对结构式报告的主观感受评价和信息完整性的评价均高于自由文本报告,且有统计学意义(P<0.001).结论胃癌结构式报告较自由文本报告内容更准确、详实及规范,更受影像科医师欢迎,临床满意度更高,具有大的应用前景.Objective To explore the feasibility and clinical value of structured report for gastric cancer preoperative staging with enhanced CT.MethodsThe enhanced CT examinations of 30 cases with gastric cancer for preoperative staging were enrolled retrospectively.All the cases include one free text report and one structured report.The diagnostic contents were compared between structured reports and free text reports,In addition,11 radiologists evaluated the convenience and educational value of both reports.And 2 surgeons of gastroenterology evaluated clinical satisfaction on the report,including subjective feeling and information integrity.Nonparametric test was used for statistical analyses.Results ltems on lesion location,enhancement characteristics cistal metastases,serosa layer description,lymph nodes evaluation and TNM staging showed good congruence between structured reports and free text reports.Items on extents of lesions showed moderate congruence,while items like overall asessment of gastricfilling,vessels evaluation and intensity characteristics of lesions showed poor congruence.The subjective feelings of radiologists on structured reports were higher than those on free text reports(P<0.05),and as for surgeons,the subjective feelings and information integrity of structured reports were both higher(P<0.001).Conclusion The structured reports of gastric cancer is more accurate,detailed and standardized than the free text reports,Therefore,with more popularity among radiologists and higher satisfaction in clinical,structured reports has great application prospects in future.
