松筋理络法联合中药外敷改善乳腺癌术后患肢肩关节活动度

摘　　要：目的观察应用松筋理络联合中药外敷法在改善乳腺癌术后患侧上肢肩关节活动度的临床效果。方法将符合纳入标准的76例患者随机分为试验组与对照组,每组各38例。对照组患者应用常规功能锻炼,试验组在此基础上,应用松筋理络法联合五味双柏散中药外敷。两组分别于治疗前、治疗后、治疗后1月±1周测量患侧上肢肩关节活动度,采用肩关节功能评价量表、肩关节constant评分量表评价上肢功能障碍程度,采用ID pain量表及麻木量表评价局部疼痛和麻木程度,主要采用重复测量方差分析对比治疗前后的差异。结果治疗后两组患者的肩关节外展、前屈的角度、上肢功能障碍程度及局部麻木情况均明显改善,且试验组改善的程度优于对照组(P<0.05)。治疗前后,两组患者的肩关节后伸角度及疼痛程度差异无统计学意义(P>0.05)。结论松筋理络法联合中药外敷法可改善乳腺癌术后肩关节活动度,从而改善上肢功能障碍患者的上肢功能,减轻局部疼痛、麻木症状,值得临床推广。Objective To observe the effect of manipulations for loosing tendons and dredging collaterals combined with external application of Chinese medicine on improving the range of the movement of the affected upper limb after breast cancer operation.Methods Seventy-six patients who met the inclusion criteria were divided into experimental group and control group according to the random number table method,with 38 cases in each group.The control group received routine functional training,while the experimental group received manipulations for loosing tendons and dredging collaterals combined with Shuangbo Powder externally on the basis of routine functional training.Before treatment,after treatment,and one month±one week after treatment,the range of movement of the affected upper limb was measured respectively.The shoulder joint function evaluation scale and the shoulder Constant scoring scale were used to evaluate the degree of upper limb dysfunction,and the ID pain scale and numbness scale were used to evaluate the degree of focal pain and numbness.Repetitive measure analysis of variance was used for the comparison of pre-and post-treatment difference.Results After treatment,shoulder abduction,flexion angle,shoulder function evaluation scale scores,constant scale scores and focal numbness scores in the 2 groups were significantly improved,and the improvement degree in experimental group was superior to that in control group(P<0.05).Before and after treatment,there was no significant difference in shoulder extension angle and pain degree between the two groups(P>0.05).Conclusion The manipulations for loosing tendons and dredging collaterals combined with external application of Chinese medicine is effective on improving the range of the movement of the affected upper limb after breast cancer operation,thus to improve the function of the affected upper limb,relieve the focal pain and numbness symptoms,which is worthy of expanding the clinical application.
