甲状腺癌患者microrna-675、LncRNA LUCAT1、Actinin-4、IGF-1R的表达与临床特征及预后的相关性

摘　　要：目的探讨甲状腺癌患者microrna-675、LncRNA LUCAT1、Actinin-4、IGF-1R的表达与临床特征及预后的相关性。方法选取2014年1月至2019年3月确诊为甲状腺癌患者78例(甲状腺癌组)与甲状腺良性结节患者78例(良性组),检测2组血液中microrna-675、LncRNA LUCAT1、Actinin-4、IGF-1R表达水平,调查、随访患者的临床特征、预后并进行相关性与影响因素分析。结果甲状腺癌组的microrna-675、LncRNA LUCAT1相对表达水平和血清IGF-1R值低于良性组,血清Actinin-4值高于良性组,差异有统计学意义(P<0.05)。随访到2020年1月1日,良性组患者都存活;甲状腺癌组中存活67例,存活率85.9%,差异有统计学意义(P<0.05)。在156例患者中,ROC曲线分析microrna-675、LncRNA LUCAT1、Actinin-4、IGF-1R鉴别诊断甲状腺癌的AUC值分别为0.843、0.807、0.811和0.837。在甲状腺癌患者中,Pearson分析显示microrna-675、LncRNA LUCAT1、Actinin-4、IGF-1R表达水平与临床分期、组织学分化、远端转移、预后都存在相关性(P<0.05);多因素Logistic回归分析显示临床分期、组织学分化、远端转移、microrna-675、LncRNA LUCAT1、Actinin-4、IGF-1R表达水平都为影响患者预后的主要因素(P<0.05)。结论甲状腺癌患者伴随有microrna-675、LncRNA LUCAT1、Actinin-4、IGF-1R的影响表达,与患者的临床特征显著相关,可用于鉴别诊断甲状腺癌,且都是影响患者预后的重要因素。Objective To investigate the correlation between of the expressions of microrna-675,LncRNA LUCAT1,Actinin-4,IGF-1R and clinical characteristics as well as the prognosis of patients with thyroid cancer.Methods A total of 78 patients with thyroid cancer who were diagnpsed and treated from January 2014 to March 2019 were enrolled as thyroid cancer group,at the same time,the other 78 patients with benign thyroid nodules were enrolled as benign case group.The expression levels of blood microrna-675,LncRNA LUCAT1,Actinin-4,IGF-1R were detected and compared between the two groups.Moreover the patients’clinical characteristics,prognosis were investigated and followed up,and the correlation and influencing factors were analyzed.Results The expression levels of microrna-675,LncRNA LUCAT1,and IGF-1R value in thyroid cancer group were significantly lower than those in benign case group,however,the serum levels of Actinin-4 were significantly higher than those in benign case group(P<0.05).After follow up to January 1,2020,all the patients in benign case group were survived,but only 67 patients in thyroid cancer group were survived,with the survival rate being 85.9%,there were significant differences between the two groups(P<0.05).Of the 156 patients,the AUC values of microrna-675,LncRNA LUCAT1,Actinin-4,and IGF-1R in the differential diagnosis of thyroid cancer were 0.843,0.807,0.811,and 0.837,respectively.Pearson analysis showed that the expression levels of microrna-675,LncRNA LUCAT1,Actinin-4,and IGF-1R in thyroid cancer group were closely correlated with clinical stage,histological differentiation,distant metastasis,and prognosis(P<0.05).Multiple factors Logistic regression analysis showed that clinical stage,histological differentiation,distant metastasis,and the expression levels of microrna-675,LncRNA LUCAT1,Actinin-4,and IGF-1R were major factors affecting the prognosis of patients(P<0.05).Conclusion The patients with thyroid cancer are accompanied by the expression of microrna-675,LncRNA LUCAT1,Actinin-4,and IGF-1R
