摘 要:目的探究乳腺癌组织转移相关蛋白(MTA)-3表达与乳腺癌腋窝淋巴结转移的相关性。方法前瞻性选取2017年5月至2020年5月西安交通大学医学部附属三二〇一医院收治的90例乳腺癌患者作为观察组,同期40例乳腺良性肿瘤患者作为对照组,检测2组患者乳腺组织中MTA-3水平,并分析MTA-3阳性与乳腺癌临床病理特征关系。根据患者有无腋窝淋巴结转移分为转移组(n=40)与非转移组(n=50),分析腋窝淋巴结转移与乳腺癌临床病理特征关系;分析MTA-3与腋窝淋巴结转移数目、Ki-67、E-cadherin的相关性。结果观察组乳腺组织中MTA-3阳性表达占33.33%,明显低于对照组的87.50%,差异有统计学意义(P<0.05)。组织学分级Ⅱ级或Ⅲ级(48.57%、22.22%)、临床分期Ⅱ期或Ⅲ期(34.62%、23.33%)、肿瘤大小>5 cm(13.33%)、有腋窝淋巴结转移(22.50%)患者MTA-3阳性率低于组织学分级Ⅰ级(100%)、临床分期Ⅰ期(62.50%)、肿瘤大小≤2 cm或2~5 cm(54.55%、34.38%)、无腋窝淋巴结转移(42.00%)患者,其他分型乳腺癌(30.49%)MTA-3阳性表达率低于三阴性乳腺癌(62.50%),差异均有统计学意义(P<0.05)。组织学分级Ⅰ级(0)、临床分期Ⅰ期(25.00%)、肿瘤大小≤2 cm及2~5 cm(18.18%、40.63%)、三阴性乳腺癌(37.50%)发生淋巴结转移占比低于组织学分级Ⅱ级或Ⅲ级(34.29%、51.85%)、临床分期Ⅱ期或Ⅲ期(38.46%、60.00%)、肿瘤大小>5 cm(60.00%)、其他分型乳腺癌患者(45.12%),差异均有统计学意义(P<0.05)。Spearman相关性分析表明,乳腺癌组织MTA-3阳性表达与腋窝淋巴结转移数目、Ki-67阳性呈负相关(r=-0.195、-0.352,P<0.05),与E-cadherin阳性表现呈正相关(r=0.239,P<0.05)。结论乳腺癌组织MTA-3阳性表达低,与腋窝淋巴结转移密切相关,在乳腺癌发生及转移中有一定的参与作用,应予以重视。Objective To investigate the correlation between the expression of metastasis associated protein(MTA)-3 and axillary lymph node metastasis in breast cancer.Methods From May 2017 to May 2020,90 breast cancer patients admitted to the 3201 Hospital,Xi'an Jiaotong University Medical School were prospectively selected as the observation group.During the same period,40 patients with benign breast tumor were selected as the control group.The level of MTA-3 in the breast tissue of the 2 groups of patients was detected.And the relationship between MTA-3 positive and the clinicopathological characteristics of breast cancer was analyzed.According to whether the patients have axillary lymph node metastasis,they are divided into metastasis group(n=40)and non-in the metastasis group(n=50),The relationship between axillary lymph node metastasis and the clinicopathological characteristics of breast cancer was analyzed;the correlation between MTA-3 and the number of axillary lymph node metastasis,Ki-67,E-cadherin was analyzed.Results The positive expression of MTA-3 in the breast tissue of the observation group accounted for 33.33%,which was significantly lower than the 87.50%of the control group,the difference was statistically significant(P<0.05).The positive expression rate of MTA-3 in histological gradeⅡorⅢ(48.57%,22.22%),clinical stageⅡorⅢ(34.62%,23.33%),tumor size>5 cm(13.33%),patients with axillary lymph node metastasis(22.50%)were lower than histological grade I(100%),clinical stageⅠ(62.50%),tumor size≤2 cm or 2 to 5 cm(54.55%,34.38%),no axillary lymph node metastasis(42.00%),the positive expression rate of MTA-3 in other types of breast cancer(30.49%)was lower than that of triple-negative breast cancer(62.50%),and the differences were statistically significant(P<0.05).The proportion of lymph node metastasis in histological gradeⅠ(0),clinical stageⅠ(25.00%),tumor size≤2 cm and 2 to 5 cm(18.18%,40.63%),triple-negative breast cancer(37.50%)were lower than histological gradeⅡorⅢ(34.29%,51.85%),clinical
