摘　　要：目的:探讨治疗前外周血炎症指标在人表皮生长因子受体2(HER-2)阳性乳腺癌患者新辅助化疗(NAC)中的作用及与预后的相关性。方法:回顾性分析我院2012年1月—2018年1月接受新辅助化疗的85例HER-2阳性乳腺癌患者的临床病理资料,根据ROC曲线,兼顾敏感性及特异性,制定外周血炎症指标最佳临界值,根据最佳临界值将患者分为高、低两组,对比不同外周血炎症指标与化疗后病理完全缓解(pCR)、无病生存期(DFS)和总生存期(OS)之间的关系。结果:中性粒细胞/淋巴细胞比值(NLR)低组患者的pCR率明显高于高NLR组的患者(P=0.038),且NLR低组较NLR高组的DFS更高(P=0.011),但中性粒细胞、淋巴细胞及NLR值的高低均与OS无明显相关性(P均>0.05)。结论:HER-2阳性乳腺癌患者NAC前的NLR值与pCR及DFS具有显著相关性,NLR值较低的患者具有较好的预后,提示NLR在HER-2阳性乳腺癌患者新辅助化疗中的预测价值。Objective:To investigate the role of peripheral blood inflammatory indicators in neoadjuvant chemotherapy of human epidermal growth factor receptor 2 positive breast cancer patients before treatment and the predictive value of prognosis.Methods:A retrospective analysis was made of the clinicopathological data of 85 patients with HER-2 positive breast cancer who received neoadjuvant chemotherapy in our hospital from January 2012 to January 2018,according to the ROC curve,sensitivity and specificity,establish the optimal critical value of peripheral blood inflammatory index,patients were divided into high and low groups according to the optimal threshold,comparing different peripheral inflammatory indexes before chemotherapy and after chemotherapy pathological complete response,relationship between disease-free survival and overall survival.Results:The pCR rate of patients with low Neutrophilto lymphocyte ratio(NLR)was significantly higher than that of patients with high NLR(P=0.038),and the DFS of patients with low NLR was higher than that of patients with high NLR(P=0.011),but there was no significant correlation between the neutrophils,lymphocytes and NLR values and OS(P>0.05).Conclusion:The NLR value before NAC in breast cancer patients with positive HER-2 was significantly correlated with pCR and DFS,and the patients with low NLR value had better prognosis,suggesting the predictive value of NLR in neoadjuvant chemotherapy in breast cancer patients with positive HER-2.
