摘　　要：目的:探讨内镜下组织胶注射或喷洒联合药物注射治疗上消化道非静脉曲张性出血(nonvariceal upper ogastrointestinal bleeding,NVUGIB)的效果。方法:按照掷骰子的方法,将本院2017年8月-2020年8月收治的118例NVUGIB患者随机分为对照组与观察组,各59例,分别给予奥美拉唑+酚磺乙胺及内镜下组织胶注射或喷洒联合药物注射治疗。对比两组临床治疗效果、手术相关指标、治疗前后实验室相关指标(红细胞比容、白细胞计数、血红蛋白水平、胃液pH值)及安全性。结果:观察组治疗总有效率显著优于对照组,差异有统计学意义(P<0.05);观察组手术相关指标均显著优于对照组(P<0.01);两组治疗后红细胞比容、血红蛋白均显著高于治疗前(P<0.05),两组治疗后白细胞计数及pH值均显著低于治疗前(P<0.05);观察组不良反应发生率显著低于对照组(P<0.05)。结论:内镜下组织胶注射或喷洒联合药物注射治疗上消化道非静脉曲张性出血的效果显著,应加以推广及应用。Objective:To investigate the effect of endoscopic tissue glue injection or spraying combined with drug injection in the treatment of nonvariceal upper gastrointestinal bleeding(NVUGIB).Method:According to the method of dice rolling,118 NVUGIB patients admitted to our hospital from August 2017 to August 2020 were randomly divided into the control group and the observation group,59 cases in each group.The control group used Omeprazole+Phenolic Sulfonamide,the observation group used tissue glue injection under endoscopy or combined drug injection spraying respectively.The clinical treatment effect,surgical indicators,laboratory indicators(RBC specific volume,WBC count,hemoglobin level and gastric pH value)before and after treatment and safety were compared between the two groups.Result:The treatment effect of the observation group was significantly better than that of the control group(P<0.05).The operative indicators in the observation group were significantly better than those in the control group(P<0.01).After treatment,erythrocyte volume,hemoglobin in both groups were significantly higher than those before treatment(P<0.05),and white blood cell counts and gastric pH level in both groups were significantly lower than those before treatment(P<0.05).The incidence adverse reactions of the observation group was significantly lower than that of the control group(P<0.05).Conclusion:Endoscopic tissue glue injection or spraying combined with drug injection is effective in treating nonvariceal upper gastrointestinal bleeding,and should be promoted and applied.
