摘　　要：目的观察3D腹腔镜Glisson蒂横断式肝切除术治疗肝癌的效果。方法选取2015年8月~2018年6月我院诊治的150例肝癌患者作为研究对象,按照随机数字表法分为对照组和观察组,每组75例。对照组行开腹肝切除术,观察组采用3D腹腔镜Glisson蒂横断式肝切除术,比较两组治疗总有效率、术后肝功能状况、机体细胞免疫功能及KPS评分。结果观察组治疗总有效率为64.00%,高于对照组的34.67%,差异有统计学意义(P<0.05)。观察组ALT、TBiL、ALB水平优于对照组,差异有统计学意义(P<0.05);观察组CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+) 优于对照组,差异有统计学意义(P<0.05);观察组KPS评分高于对照组[(90.99±13.32)分vs(73.98±12.03)分],差异有统计学意义(P<0.05)。结论3D腹腔镜Glisson蒂横断式肝切除术能够改善肝癌患者肝功能,提高患者机体免疫功能及体力状况,具有应用有效性。Objective To observe the effect of 3D laparoscopic Glisson pedicle transverse hepatectomy in the treatment of patients with liver cancer.Methods A total of 150 liver cancer patients diagnosed and treated in our hospital from August 2015 to June 2018 were selected as the research objects,and they were divided into a control group and an observation group according to the random number table method,with 75 cases in each group.The control group underwent open hepatectomy,and the observation group underwent 3D laparoscopic Glisson pedicle hepatectomy to compare the total effective rate of treatment,postoperative liver function status,body cell immune function and KPS score between the two groups.Results The total effective rate of treatment in the observation group was 64.00%,which was higher than 34.67%in the control group,the difference was statistically significant(P<0.05).The levels of ALT,TBIL and ALB in the observation group were better than those in the control group,the difference was statistically significant(P<0.05);The observation group CD4^(+),CD8^(+),CD4^(+)/CD8^(+) was better than the control group,the difference was statistically significant(P<0.05);The KPS score of the observation group was higher than that of the control group[(90.99±13.32)scores vs(73.98±12.03)scores],the difference was statistically significant(P<0.05).Conclusion 3D laparoscopic Glisson pedicle cross-section hepatectomy can improve the liver function of patients with liver cancer,improve the immune function and physical status of the patient,and is effective.
