摘 要:目的研究不同刺激模式诱致大鼠坐骨神经痛中星形胶质细胞作用机制。方法取SD大鼠100只,随机分组后取大鼠自体尾部椎间盘组织对大鼠L5神经根干扰造模,分为椎间盘压迫(compression,CP)组,无压迫的间盘移植(disc,DC)组、髓核移植(nucleus pulposus,NP)组、纤维环移植(annulus fibrosus,AF)组和假手术(Sham)组,每组20只大鼠。设定时间(术前1天,术后3天、7天、21天)测定大鼠双侧后足底机械刺激缩足阈值(paw withdrawal mechanical threshold,PWMT)。同时脊髓取材行胶质纤维酸性蛋白(glial fibrillary acidic protein,GFAP)抗体免疫组织化学染色,观察并统计星形胶质阳性细胞数及其平均光密度值(optical density value,OD)。结果(1)与术前基础疼痛阈值相比较,实验组术后21天内后足底PWMT明显降低,而对照组无显著变化;(2)与CP组相比,DC组、AF组及NP组术后均表现出显著的痛觉过敏,差异有统计学意义(P<0.05);(3)除Sham组外,各组术后各时间点的脊髓免疫组织化学染色见GFAP细胞计数较术前明显增多[CP组:术前(6.67±1.76),术后3天(9.13±2.26),术后7天(13.67±4.89),术后21天(11.57±3.42);DC组:术前(6.50±1.19),术后3天(16.11±4.01),术后7天(19.64±3.63),术后21天(18.51±3.17);NP组:术前(6.83±1.84),术后3天(13.82±2.83),术后7天(17.45±1.66),术后21天(15.68±1.27);AF组:术前(6.51±1.58),术后3天(10.67±2.16),术后7天(15.83±2.71),术后21天(13.53±2.35)],OD增大[CP组:术前(0.058±0.03),术后3天(0.12±0.01),术后7天(0.17±0.04),术后21天(0.10±0.02);DC组:术前(0.057±0.03),术后3天(0.22±0.03),术后7天(0.48±0.04),术后21天(0.28±0.03);NP组:术前(0.058±0.03),术后3天(0.19±0.02),术后7天(0.32±0.03),术后21天(0.17±0.02);AF组:术前(0.058±0.02),术后3天(0.15±0.03),术后7天(0.21±0.03),术后21天(0.12±0.03)],其中术后7天增多最为显著,而对照组少有GFAP细胞的阳性表达;(4)实验组中大鼠后足PWMT和GFAP阳性细胞计数及OD均呈负相关�Objective To explore activation of astrocytes in different stimulus patterns of discogenic sciatic pain in rats.Methods Sprague-Dawley rats(n=100)were randomly divided into compression group(CP Group;L5 dorsal root compression;n=20),non-compression disc group(DC Group;n=20),non-compression nucleus pulposus group(NP group;n=20),non-compression annulus fibrosis group(AF group;n=20)and sham group.Paw Withdrawal Mechanical Threshold(PWMT)were determined 1 day preoperatively,3 days,7 days and 21 days postoperatively.Immunohistochemically staining of glial fibrillary acidic protein(GFAP)antibody was conducted to observe astrocytes and optical density value.Results(1)PWMT of the posterior foot on the operation side significantly decreased compared with the preoperative pain threshold in experimental groups within 21 days after operation,especially in the DC group.However,no significant changes had been found in the sham group;(2)Rats in all other groups(DC group,AF group,NP group)showed significant hyperalgesia compared with that of the CP group(P<0.05);(3)GFAP positive cells increased significantly than that before operation in all experimental groups as presented by immunohistochemical staining[CP group:(6.67±1.76)before surgery,(9.13±2.26)3 days after surgery,(13.67±4.89)7 days after surgery,(11.57±3.42)21 days after surgery;DC group:(6.50±1.19)before surgery,(16.11±4.01)3 days after surgery,(19.64±3.63)7 days after surgery,(18.51±3.17)21 days after surgery;NP group:(6.83±1.84)before surgery,(13.82±2.83)3 days after surgery,(17.45±1.66)7 days after surgery,(15.68±1.27)21 days after surgery;AF group:(6.51±1.58)before surgery,(10.67±2.16)3 days after surgery,(15.83±2.71)7 days after surgery,(13.53±2.35)21 days after surgery].The mean optical density value increased significantly than that before operation[CP group:(0.058±0.03)before surgery,(0.12±0.01)3 days after surgery,(0.17±0.04)7 days after surgery,(0.10±0.02)21 days after surgery;DC group:(0.057±0.03)before surgery,(0.22±0.03)3 days after su
