老年非小细胞肺癌患者三维适形放疗联合化疗的疗效及影响因素分析

摘　　要：目的:研究老年非小细胞肺癌(NSCLC)患者三维适形放疗联合化疗的疗效及影响因素。方法:回顾性选择2015年1月~2018年10月期间在某院接受三维适形放疗联合化疗的58例局部晚期NSCLC老年患者,评价短期疗效及2年生存率,分析临床病理特征、PCNA及Ki-67表达与疗效的关系。结果:NSCLC患者的治疗缓解率为65.52%、2年总生存率(OS)为46.55%;不同性别、年龄、病灶部位NSCLC患者的治疗缓解率、2年OS无明显差异(P>0.05);不同AJCC分期、病理类型、PCNA及Ki-67表达的NSCLC患者比较,治疗缓解率、2年OS、生存曲线有显著性差异(P<0.05);AJCC分期、病理类型、PCNA及Ki-67表达是治疗缓解率的独立影响因素,而AJCC分期、PCNA及Ki-67表达是2年OS的独立影响因素。结论:局部晚期NSCLC老年患者三维适形放疗联合化疗的疗效与AJCC分期、病理类型、PCNA及Ki-67表达有关。Objective:To study the efficacy and influencing factors of three-dimensional conformal radiotherapy combined with chemotherapy in elderly patients with non-small cell lung cancer(NSCLC).Methods:58 elderly patients with locally advanced NSCLC who received three-dimensional conformal radiotherapy combined with chemotherapy in a hospital from January 2015 to October 2018 were retrospectively selected.The short-term efficacy and 2-year survival rate were evaluated.The correlation among clinical pathological characteristics,PCNA,Ki-67 expression,and efficacy was analyzed.Results:The treatment remission rate of NSCLC patients was 65.52%,and the 2-year overall survival rate(OS)was 46.55%.There was no significant difference in the treatment remission rate and 2-year OS of NSCLC patients with different gender,age and lesion location(P>0.05).The treatment remission rate,2-year OS and survival curve of NSCLC patients with different AJCC stages,pathological types,PCNA and Ki-67 expression were significantly different(P<0.05).AJCC stage,pathological type,PCNA and Ki-67 expression were independent influencing factors of remission rate,while AJCC stage,PCNA and Ki-67 expression were independent influencing factors of 2 years OS.Conclusion:The efficacy of three-dimensional conformal radiotherapy combined with chemotherapy in elderly patients with locally advanced NSCLC correlates with AJCC stage,pathological type,PCNA and Ki-67 expression.
