腹腔镜低位直肠癌保肛术中经肛门结肠直肠套入式吻合对患者肠道功能的影响

摘　　要：目的:探讨经肛门结肠直肠套入式吻合对腹腔镜低位直肠癌保肛术患者肠道功能的影响。方法:将某院2018年1月~2019年9月期间79例行腹腔镜低位直肠癌保肛术的患者分组,按照掷硬币法分为对照组39例给予经肛门结肠肛管吻合术(改良Parks手术)治疗,观察组40例给予经肛门结肠直肠套入式吻合治疗,观察两组患者手术情况、肛门控便情况、术前及术后8个月时肠道功能情况和术后并发症发生情况。结果:两组患者手术、住院、术后肛门排气时间、术中出血量及术后并发症情况均无明显统计学差异(P>0.05);两组患者术前美国纽约纪念斯隆-凯特林癌症中心(MSKCC)肠道功能问卷各维度得分无明显差异(P>0.05);观察组术后3个月肛门控便情况优于对照组,且术后8个月时MSKCC肠道功能、便频便急及排便感觉异常的评分均高于对照组(P<0.05)。结论:采用经肛门结肠直肠套入式吻合保肛术的患者可对肠道功能及肛门控便功能进行有效的保护。Objective:To investigate the effect of transanal colorectal intubation anastomosis on intestinal function in patients undergoing aparoscopic anus preserving surgery for low rectal cancer.Methods:From January 2018 to September 2019,79 patients with low rectal cancer who underwent laparoscopic anus preserving surgery in a hospital were divided into two groups.According to the coin tossing method,39 patients in the control group were given transanal coloanal anastomosis(modified Parks operation),and 40 cases in the observation group were treated with transanal colorectal intubation anastomosis.The operation,anal defecation control,intestinal function before and 8 months after operation and postoperative complications were observed in the two groups.Results:There was no significant difference in operation,hospitalization,postoperative anal exhaust time,intraoperative blood loss and postoperative complications between the two groups(P>0.05).There was no significant difference in the scores of each dimension of New York Memorial Sloan Kettering Cancer Center(MSKCC)intestinal function questionnaire between the two groups before operation(P>0.05).The anal defecation control of the observation group was better than that of the control group at 3 months after operation,and 8 months after operation,the MSKCC scores of frequent defecation,urgency defecation and abnormal defecation sensation were higher than those in the control group(P<0.05).Conclusion:Transanal colorectal intubation anastomosis can effectively protect the intestinal function and anal defecation control function of patients.
