成人依恋类型与产后抑郁症状的关联性研究

摘　　要：目的:研究成人依恋类型与产后抑郁症发病风险之间的相关性。方法:根据入排标准,选择产后42 d回院进行产后保健的产妇1366人,按照自愿原则进行一般资料的收集及爱丁堡产后抑郁量表、成人依恋量表问卷的填写。结果:1351名产妇纳入最终分析,其中496人检出有产后抑郁症状,发生率为36.7%。对有产后抑郁症状组和无产后抑郁症状组各成人依恋类型发生比例差异均有统计学意义(P<0.001)。在校正年龄、学历和职业之后,安全型(AOR=1.00)、先占型(AOR=5.84,95%CI:3.70~7.69)、拒绝型(AOR=8.87,95%CI:4.76~16.20)、恐惧型(AOR=16.86,95%CI:6.44~43.27)均增加了产后抑郁症状的患病风险。结论:成人依恋类型与产后抑郁症状的发生相关。Objective:To study the correlation between adult attachment styles and the risk of postpartum depression.Method:According to the inclusion and exclusion criteria,1366 women with postpartum health care who returned to hospital for 42 days were asked for participating in the study.Demographic characteristics were collected and Edinburgh Postpartum Depression Scale and Adult Attachment Styles Scale were completed on a voluntary basis.Results:1351 women were included in the final data analysis.496 of them were found to have postpartum depression,with a prevalence of 36.7%.The differences in the scores and prevalence of adult attachment styles between the groups with and without postpartum depression were statistically significant(P<0.001).After adjusting for age,education and occupation,secure style(AOR=1.00),preoccupied style(AOR=5.84,95%CI:3.70-7.69),dismissing style(AOR=8.87,95%CI:4.76-16.20),fearful style(AOR=16.86,95%CI:6.44-43.27)increased the risk of postpartum depression.Conclusion:Adult attachment styles are associated with postpartum depression.
