流式细胞术在NK细胞肿瘤诊断中的应用并文献复习

摘　　要：目的侵袭性NK细胞白血病和结外NK/T细胞淋巴瘤-鼻型是成人最主要的EBV病毒相关的T细胞和NK细胞增殖性疾病,目前针对NK细胞的检测手段,主要包括细胞形态学、免疫组织化学,以及流式细胞术,本文主要探讨流式细胞术的优越性。方法本文报道2例成人NK细胞肿瘤的诊断及治疗过程,其中1例患者合并继发性噬血细胞综合征。结果流式细胞术的应用在他们的诊断中发挥了重大作用。结论单纯细胞形态学难以判断NK细胞克隆性。病理免疫组织化学通常被认为是诊断"金标准",但是通常NK细胞恶性肿瘤患者呈侵袭性病程,可能无法耐受活检手术或者组织取材不满意而使得诊断证据不足。流式细胞术可以尽可能多地、快速地获取靶向细胞,并进行细胞大小、胞浆颗粒度及分化抗原的综合高效分析。Objective Aggressive NK-cell leukemia and extranodal NK/T-cell lymphoma,nasal type are the most important EBV virus-related T cell and NK cell proliferative diseases in adults.The current detection methods for NK cells mainly include cell morphology,immunohistochemistry,and flow cytometry.This article is to discusses the advantages of flow cytometry.Methods This article reports on the diagnosis and treatment of two adult NK cell tumors,one of which has a secondary hemophagocytic lymphohistiocytosis syndrome.Results The application of flow cytometry has played a major role in their diagnosis.Conclusion Using cell morphology alone is difficult to judge the clonality of NK cells.Pathological immunohistochemistry is generally regarded as the“gold standard”for the diagnosis,but patients with NK cell malignancy usually have an aggressive course and may not be able to tolerate biopsy surgery or unsatisfactory tissue selection,which makes the diagnostic evidence insufficient.Flow cytometry can obtain as many targeted cells as possible and can be conducted quickly,and perform comprehensive and efficient analysis of cell size,cytoplasmic granularity,and differentiation antigen.
