锐扶刀治疗宫颈上皮内瘤变患者的临床效果

摘　　要：目的比较锐扶刀和传统冷刀锥切治疗宫颈上皮内瘤变(CIN)患者中的临床意义和价值。方法选取2020年1-11月期间在该院诊断为CIN1~CIN3,且病理活检结果排除CIN3累及腺体患者95例患者为研究对象。随机将95例入组患者分为观察组和对照组,其中观察组使用锐扶刀治疗,共纳入41例患者。对照组采用传统冷刀锥切手术,共纳入54例。统计分析比较两组手术时间、出血量、手术前后病理相符情况率、切缘阳性率及切缘厚度。结果对照组的手术时间长于观察组(P<0.001);对照组的术中出血量多于观察组(P<0.001);对照组的切缘厚度大于观察组(P<0.001)。两组的手术前后病理相符率比较差异均无统计学意义(均P>0.05)。结论对于未累及腺体的CIN患者,采用冷刀锥切术与锐扶刀的治疗效果基本相同。但锐扶刀能够显著降低了手术时间和出血量,且减少了不必要的宫颈损伤,具有重要的临床意义。Objective To compare the clinical significance and value of radio-frequency knife and cold-knife conization in treatment of patients with cervical intraepithelial neoplasia(CIN).Methods From January to November in 2020,95 patients diagnosed as CIN 1-CIN 3(excluding CIN 3 with gland involvement by biopsy)were selected and randomly divided into observation group and control group,41 patients in observation group were treated by radio-frequency knife,54 patients in control group were treated by cold-knife conization.The operation time,the amounts of bleeding,the pathological coincidence rates before and after operation,the positive rates of incision margin,and thicknesses of incision margin in the two groups were analyzed statistically and compared.Results The operation time in control group was statistically significantly longer than that in observation group(P<0.001);the amount of intraoperative bleeding in control group was statistically significantly higher than that in observation group(P<0.001);the thickness of incision margin in control group was statistically significantly thicker than that in observation group(P<0.001);there was no statistically significant difference in the pathological coincidence rate before and after operation between the two groups(P>0.05).Conclusion For CIN patients without gland involvement,the curative effects of cold-knife conization and radio-frequency knife are similar,but radio-frequency knife can significantly reduce operation time,amount of intraoperative bleeding,and unnecessary cervical injury,which is of important clinical significance.
