摘  要:目的探讨卒中后抑郁患者用药前肿瘤坏死因子-α(TNF-α)水平及其与认知功能的相关性。方法观察组:卒中后半年内抑郁发作的患者42例。对照组:健康成人40例。应用ELISA法分别对观察组和对照组进行TNF-α血清水平的检测;采用剑桥神经心理测试(CANTAB)中的工作记忆和策略运用(SWM),分别对观察组和对照组进行认知功能测试,内容包括组内搜索错误、组间搜索错误、策略运用和平均反应时间。结果卒中后抑郁患者血清TNF-α水平高于对照组,差异有统计学意义(P<0.05);认知测试(SWM)中,观察组的组内搜索错误、组间搜索错误观察组、策略运用观察组均高于对照组,差异有统计学意义(P<0.05);相关性分析发现,观察组血清TNF-α水平与SWM组内搜索错误、组间搜索错误、策略运用和平均反应时间均呈正相关(P<0.01)。结论本研究发现,卒中后抑郁患者血清TNF-α水平升高、认知功能下降,提示TNF-α可能参与了卒中后抑郁的发生,并参与介导了认知功能下降。Objective To explore the characteristics of TNF-α level before medication in patients with poststroke depression and its correlation with cognitive function. Methods Observation group :42 patients with depressive episodes within six months after stroke. Control group :40 healthy adults. ELISA method was used to detect the serum levels of TNF-α in the observation group and the control group;the working memory and strategy application(SWM)in the Cambridge Neuropsychological Test(CANTAB)were used to test the cognitive function of the observation group and the control group.,The content includes search errors within groups,search errors between groups,strategy application and average reaction time. Results The serum TNF-α level of patients with post-stroke depression was higher than that of the control group,and the difference was statistically significant(P<0.05). In the cognitive test(SWM),the search error within the observation group,the search error between the observation groups. The strategy application observation group was higher than the control group,the difference was statistically significant(P<0.05). Correlation analysis found that the observation group serum TNF-α level and SWM group search error,search error between groups,strategy application and average response Time was positively correlated(P<0.01). Conclusion This study found that patients with post-stroke depression increased serum TNF-α levels and decreased cognitive function,suggesting that TNF-α may be involved in the occurrence of post-stroke depression and mediate cognitive decline.
