摘 要:目的观察神经生长因子(NGF)、白细胞介素-4(IL-4)、干扰素-γ(IFN-γ)、白细胞介素-1β(IL-1β)在原发性肝癌患者中的水平及与预后的关系。方法选取80例原发性肝癌患者作为肝癌组,另选取80例健康体检无异常者作为健康组,80例非肿瘤的肝脏疾病(包括甲肝、乙肝、肝囊肿等)患者作为肝病组。检测并比较三组血清NGF、IL-4、IFN-γ、IL-1β水平,比较肝癌组不同预后患者治疗前的血清NGF、IL-4、IFN-γ、IL-1β水平。结果肝癌组患者血清NGF(35.58±3.72)ng/L、IL-4(122.08±22.76)ng/L、IL-1β(21.31±2.36)pg/ml均高于肝病组的(18.39±2.76)ng/L、(65.43±7.73)ng/L、(19.05±1.43)pg/ml和健康组的(11.39±0.76)ng/L、(48.05±5.73)ng/L、(11.24±0.91)pg/ml,且肝病组高于健康组,差异有统计学意义(P<0.05);肝癌组患者的IFN-γ(66.24±7.38)pg/ml低于肝病组的(89.91±10.86)pg/ml和健康组的(132.40±14.73)pg/ml,且肝病组低于健康组,差异有统计学意义(P<0.05)。肝癌组患者经治疗71例好转存活,9例恶化死亡。治疗前,好转存活患者的NGF(32.55±3.72)ng/L、IL-4(119.47±20.40)ng/L、IL-1β(20.76±2.16)pg/ml均低于恶化死亡患者的(59.45±8.33)ng/L、(142.65±17.85)ng/L、(25.69±3.33)pg/ml,IFN-γ(67.94±6.84)pg/ml高于恶化死亡患者的(52.86±9.67)pg/ml,差异有统计学意义(P<0.05)。结论原发性肝癌患者的血清NGF、IL-4、IFN-γ、IL-1β水平发生变化,因此,在疑似患者的诊断中对其血液进行NGF联合IL-4、IFN-γ、IL-1β检测,可根据检测结果对患者的疾病进行早期诊断,并可根据其结果对预后进行预测,为临床治疗方案的选择提供参考资料。Objective To observe the correlation between nerve growth factor(NGF),interleukin-4(IL-4),interferon-γ(IFN-γ),interleukin-1β(IL-1β)and prognosis in patients with primary liver cancer.Methods There were 80 patients with primary liver cancer as liver cancer group,80 healthy subjects as healthy group,and 80 patients with non-tumor liver diseases(including hepatitis A,hepatitis B,liver cysts)as the liver disease group.The levels of NGF,IL-4,IFN-γand IL-1βof the three groups were detected and compared,and the serum NGF,IL-4,IFN-γ,IL-1βlevels before treatment in patients with different prognosis in liver cancer group were compared.Results The serum NGF(35.58±3.72)ng/L,IL-4(122.08±22.76)ng/L,IL-1β(21.31±2.36)pg/ml of liver cancer group were higher than liver disease group[(18.39±2.76)ng/L,(65.43±7.73)ng/L,(19.05±1.43)pg/ml]and healthy group[(11.39±0.76)ng/L,(48.05±5.73)ng/L,(11.24±0.91)pg/ml],and the liver disease group was higher than healthy group.All the difference was statistically significant(P<0.05).The IFN-γ(66.24±7.38)pg/ml of liver cancer group was lower than(89.91±10.86)pg/ml of liver disease group,and(132.40±14.73)pg/ml of healthy group,and liver disease group was lower than healthy group.All the difference was statistically significant(P<0.05).After treatment,71 patients in the liver cancer group improved and survived,and 9 patients worsened and died.Before treatment,the NGF(32.55±3.72)ng/L,IL-4(119.47±20.40)ng/L,IL-1β(20.76±2.16)pg/ml of survival patients were lower than(59.45±8.33)ng/L,(142.65±17.85)ng/L,(25.69±3.33)pg/ml of death patients,and IFN-γ(67.94±6.84)pg/ml was higher than(52.86±9.67)pg/ml of death patients.All the difference was statistically significant(P<0.05).Conclusion There is a change in the level of NGF,IL-4,IFN-γand IL-1βin patients with primary liver cancer.Therefore,in the suspected diagnosis of patients,the detection of NGF combined with IL-4,IFN-γ,IL-1βin their blood can make early diagnosis of the disease,and predict the prognosis according to the
