闭角型青光眼合并白内障应用超声乳化联合小梁切除术治疗的效果分析

摘　　要：目的对比分析应用超声乳化联合小梁切除术治疗闭角型青光眼合并白内障的治疗效果。方法74例(74眼)闭角型青光眼合并白内障患者,随机分为对照组和观察组,各37例。对照组应用单纯小梁切除术治疗,观察组应用超声乳化联合小梁切除术治疗。对比两组患者术后并发症发生情况以及手术前后视力和眼压。结果观察组术后1个月并发症发生率为5.41%,低于对照组的24.32%,差异具有统计学意义(P<0.05)。术后1个月,观察组患者眼压及视力水平分别为(12.19±1.38)mm Hg(1 mm Hg=0.133 kPa)、(4.95±0.53),均优于对照组的(24.18±1.82)mm Hg、(4.62±0.41),差异均具有统计学意义(P<0.05)。结论闭角型青光眼合并白内障应用超声乳化联合小梁切除术治疗可以减少并发症的发生,改善患者视力,降低患者眼压,安全性高,适宜临床推广应用。Objective To compare and analyze the effect of phacoemulsification combined with trabeculectomy in the treatment of angle-closure glaucoma with cataract.Methods A total of 74 cases(74 involved eyes)of angle-closure glaucoma with cataract were randomly divided into control group and observation group,with 37 cases in each group.The control group was treated with trabeculectomy,and the observation group was treated with phacoemulsification combined with trabeculectomy.The occurrence of postoperative complications,visual acuity and intraocular pressure before and after operation were compared between the two groups.Results The incidence of postoperative complications of the observation group was 5.41%,which was lower than 24.32%of the control group,and the difference was statistically significant(P<0.05).1 month after operation,the intraocular pressure and visual acuity of the observation group were(12.19±1.38)mm Hg(1 mm Hg=0.133 kPa)and(4.95±0.53),which were better than(24.18±1.82)mm Hg and(4.62±0.41)of the control group,and the difference was statistically significant(P<0.05).Conclusion Combination of phacoemulsification and trabeculectomy in the treatment of angle-closure glaucoma with cataract can reduce the incidence of complications,improve the visual acuity,reduce intraocular pressure.It is safe and suitable for clinical promotion and application.
