腹腔镜腹膜后与开放性肾癌根治术治疗局限性肾癌的对比分析

摘　　要：目的:探讨腹腔镜腹膜后与开放肾癌根治术治疗局限性肾癌的临床疗效。方法:纳入2018年8月至2019年8月收治的72例局限性肾癌患者,按手术方式分为两组,对照组(n=36)行开放性肾癌根治术,观察组(n=36)行腹腔镜腹膜后肾癌根治术。对比分析两组治疗效果。结果:观察组术中出血量、手术时间、肿瘤根治性优于对照组,差异有统计学意义(P<0.05);观察组并发症发生率、术后血清皮质醇、C反应蛋白低于对照组,术后肛门排气时间、排便时间、下床活动时间、住院时间短于对照组,差异有统计学意义(P<0.05)。观察组术后轻度疼痛比例、生存质量评分高于对照组,差异有统计学意义(P<0.05)。结论:与开放性肾癌根治术相比,腹腔镜腹膜后肾癌根治术治疗局限性肾癌的效果更优越。Objective:To study the effect of retroperitoneal laparoscopic and open radical nephrectomy for localized renal carcinoma.Methods:Seventy-two patients with localized renal cell carcinoma from Aug.2018 to Aug.2019 were enrolled in this study and were divided into two groups according to different surgical treatment methods.36 patients in the control group were treated with open radical nephrectomy,and 36 cases in the observation group were treated with retroperitoneal laparoscopic radical nephrectomy.The therapeutic effects of the two groups were compared.Results:The intraoperative blood loss of the observation group was less than that of the control group,the operation time was shorter than that of the control group,and the effect of radical cure was better than that of the control group,the differences were statistically significant(P<0.05).The incidence of complications,postoperative serum cortisol and C-reactive protein in the observation group were lower than those in the control group,and the postoperative anal exhaust and defecation time,ambulation time and hospital stay in the observation group were less than those in the control group(P<0.05).The proportion of postoperative mild pain and survival quality score of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).Conclusions:Compared with open radical nephrectomy,retroperitoneal laparoscopic radical nephrectomy is more effective in the treatment of localized renal carcinoma.
